THIS VISIT - TANK SET - OBSERVED THE FOLLOWING:
ON -SITE at approx. 9:30 AM on 4/9/2018 with Michael D. Zarrella/ President/ Wilson's
Petroleum Equipment, Inc. and Wawa's Third -Party inspector Kelly Pugh with Crompco.
TANKS: (3) Xerxes fiberglass double wall Brine Filled Interstitial USTs (EQ-200).
Tank Alias #1: North Compartmental UST, 22, 000 gallons (12k DSL /10k HUL), UL B-53332171.
Tank Alias #2: Middle UST, 20,000 gallon Regular; UL #B-53332266.
Tank Alias #3: South Compartmental UST, 22, 000 gallons (12k E-Free /10k LUL), UL B-
53332270.
*Wawa calls the compartment tank: Tank #1 (#1 ) & Tank #3 (#3 & #5).
TANK ARRIVAL CONDITION/ BRINE LEVELS: USTs arrived earlier with the brine levels they left
the factory with per Mike Zarrella/ President/ Wilson's Petroleum Equipment.
At arrival tanks were inspected for any damage, lifted and safely staged on piles of pearock.
UST TESTING BEFORE BURIAL: Earlier this morning Wilson's pressure & soap tested the tanks.
:•The primaries were carefully Air Tested at 3psi; no brine leaks were observed inside or outside
tanks; no air leaks at fittings and manways observed per Kelly Pugh and Mike Zarrella.
TANK PIT PREPARATION:
- Dry -Hole Installation: UST excavation sheet piled and dewatered (NPDES permitted).
- Tank hole well prepped; lined with geotextile fabric and 12-inch-thick tank bed of DOT approved
crushed stone; leveled flat & center marks painted where the tanks will sit.
- Deadmen set evenly spaced and aligned in hole to coincide with the tanks straps.
- Perimeter of tank pit cordoned off with safety railing.
- Tanks burial depths and levels were measured.
DEADMEN: contractor made per Xerxes requirements; 12" X 18" with rebar anchor points.
TANK SET:
- (3) USTs already set into tank hole just before my arrival.
- Each tank had its own set of deadmen; deadmen were doubled between USTs; deadmen set the
length of the tanks.
All deadmen were observed outside of the tank shadows.
- Deflections of the tanks were taken and recorded per Xerxes requirements.
Manufacture supplied FRP tank anchor straps installed at the Xerxes pre -marked
locations; no deadmen anchor points shared with a second anchor strap.
Tanks 3+ feet apart to assure they're equal to or greater than the width of the 2 deadmen.
After the anchor straps were tightened, the third tank diameter measurement was taken to
assure tank deflection was within allowable limits.
- All the galvanized turnbuckles were coated for corrosion prevention:
- Anode Bags set in tank pit and wired to deadmen turnbuckles to provide added Cathodic
Protection.
- Ample spacing (>18") between the sidewalls, tank end caps and the sides of the excavation.
- A mountain of clean DOT grade sized crushed stone (1/8th" & 1/2") onsite for backfill.
- Wilson's men in tank pit carefully tamping backfill under the tank body and domes with custom
made PVC tamping rods.
- Backfilling process continuously monitored.
- Tanks to be ballasted with city water.

THIS VISIT - Piping Air Tests/ Soap Tests and Containment Hydrostatic Tests.
ON -SITE at approx. 9:OOAM on 7/16/2018 with Mark Spiegel / Wilsons Petroleum
Equipment; I observed the following.
PRODUCT PIPING: 2" Franklin Fueling Systems/ APT XP-200-SC Series (EQ-618). Light blue,
double walled flex piping; installed inside APTs 4" light blue corrugated. chase pipe.
The APT ducting is not approved containment, but it is air -testable.
Product piping beautifully laid out; no tight bend radiuses; no piping in contact with each other; all
piping entries at or near 90-degree perpendicular to the sump walls.
- Burial Depth: product piping observed greater than the minimum required depth (6" of backfill
above the pipe + 6" of concrete).
> Piping Slope: grade down to the tank field achieved.
- ALL product piping and electrical conduit pipe entries closely observed; all Duct Pipe Boots fitted
tightly over the Rigid Entry Fittings, all boot pipe straps found tight.
VENT PIPING: NOV Fiberglass Systems Dualoy 3000/LCX, (EQ-291).
- All Vent piping downhill to tank tops.
- Below -ground steel vent piping connections to the underground fiberglass, all coated
for corrosion protection.
PIPE TRENCH: Beautifully prepared per manufacture instructions. - Trench sufficient width to
provide the minimum of 6" of space between the outside of a piping run and the trench wall.
.Lined with geotextile fabric and at least a 6" bed of clean crushed rock under the piping.
'Required 4" spacing between piping maintained by using short scrap pieces of the 4" chase pipe.
BACKFILL: Clean, DOT Grade, crushed rock (1/8 inch to 112 inch) onsite for backfill.
STP SUMPS & FILL SUMPS; (10 total) (5) yellow fiberglass STP Sumps and (5) yellow fiberglass
Fill Sumps holding the Spill Buckets and Vapor Recovery Buckets.
The plans EQ's show Mott -Smith Consulting Group, LLC/ Fibrelite S8CR-FL100MP (EQ-776)
Multi -Port Spill containment manhole covers (EQ-859) to try to prevent storm water intrusion.
Sumps nicely epoxied to tank collars and glued with 3 layers of fiberglass & resin; surfaces
properly sanded prior to fiberglass work. All fiberglass resin work appeared uniform.
- Tops of sumps also glued with 3 layers of fiberglass & resin.
- Sump product pipe Entry Fittings: are APT Rigid Entry Fittings (made of Glass -filled nylon; are
UL approved) model RDEB-200-SC; the sump & pipe seal material is Nitrile 1502. The Rigid
Fittings also include Nitrile rubber flex boots, which are clamped onto the fitting and double
clamped over the pipe duct; the design prevents chase pipe from entering the sumps.
— I checked all chase pipe clamps to assure they were tight.
Sump Electrical Conduit Entry Fittings; are APT Flexible nitrile Entry Fittings.
DISPENSER LINERS: (8) dark blue fiberglass liners; are APT/Franklin Fueling Systems dark blue
fiberglass dispenser liners, model DHW-3617, (EQ-786).
UST INTERSTITIAL SENSOR SUMPS (4) yellow fiberglass OPW Flexworks fiberglass sumps
(EQ-368.) - Labels on sumps were DPW S45; might be Mott -Smith Consulting Group, LLC/
Fibrelite Composites, Ltd., (EQ-776).
PRODUCT PIPING AIR & SOAP TESTS: - Proper Air -test Gauges used (testing psi at gauge
midrange)
Primary and secondary piping air test gauges observed; primary's at 50 psi, secondary's at 5 psi;
all pressures had held for three days per Romel Dori / Installer/ Wilsons Petroleum.
- Piping fittings all soap -tested, no air leak or bubbles were observed.
- Vent Piping air tested at 5 psi; soap tested and passed per Romel Dori.
HYDROSTATIC TESTs: (STP Sumps, Fill Sumps, Dispenser Liners, Annular Sensor Sumps and
Spill & Vapor Recover Buckets)
- ALL Sumps Hydro tested to tops of sumps at Wawa's request; no water loss detected in any

containment.
- Spill Buckets & Vapor Recover Buckets Hydro tested, no water loss observed.
- All outside containment penetration fittings inspected, no water leaking, marked hydro levels
indicated no loss.
- Containments filled yesterday afternoon per Romel'Dori.

FINAL INSPECTION
ON -SITE at approx. 10:30 AM on 8/24/2018 with Louie DeLauter/ Installer/ Wilsons Petroleum
Equipment; I inspected the following:
(5) STP SUMPS:
Submerged Turbine Pumps (STPs) are Franklin Fueling Systems (Formerly FE
Petro, Inc.) with Mechanical Line Leak Detector models 403168901 STP-MLD+G for gas &
403170901 STP-MLD+D for diesel, (EQ-335).
Sumps observed clean & dry, no fuel leaks found.
No Flex Connectors used; allowed with Flex Pipe.
All product piping secondary test boots are loosened allowing the seconadrys to be open.
All sump covers with intact rubber seals; all inside sump cover locking levers work well.
Backfill the correct level around sump rims.
Green -Wire at sump rims for testing sacrificial anodes installed on the deadmen anchoring
hardware.
- STP Sump Sensors: INCONTm brand (sold by Franklin Fuel Systems) FMP-DTS is a
discriminating turbine sump BriteT"' sensor; (EQ-456).
- Sump Sensors nicely slide into position at sump floor inside short lengths of fixed 3-inch
fiberglass piping; a superior sensor mounting method.
All electrical junction boxes closed.
(8) DISPENSERS: - All liners observed clean & dry, no fuel leaks found.
All dispenser properly anchored with large hardware (4 per disp.).
Flex Connectors installed, connected to bottom of the Shock -Valves.
All secondary test ports air tubes cut off/ all secondary's open in the liners (and the STP Sumps).
Shear Valves and Cross bar hardware all inspected and anchored tight.
Dispenser Sump Sensors: INCONTm brand FMP-DDS, now Franklin Fueling Systems (Formerly
Incon Intelligent Controls, Inc.); (EQ-456.)
- Sensors fixed in position at sump floor with manufacture -supplied steel pipe -strap hardware. An
unfortunate design because the hardware rusts away and when annually tested, the sensors often
are not re -hung properly or are not repositioned properly, ending up several inches of the sump
floor.
OVERFILL PROTECTION: Fill Port Drop Tubes with OPW 71 SO-410C Overfill Prevention
Flapper
Valves (EQ-697). Also equipped with Veeder-Root Overfill Alarm model 790091-001 with Alarm
Acknowledgement Switch 790095-001. (EQ-877).
OVERSPILL PROTECTION: (5) SPILL BUCKETS inside FILL SUMPS:
- All sumps and buckets observed clean & dry, all Fill Port Caps tight and locked.
-FILL Sump Sensors are INCONTm brand (sold by Franklin Fuel Systems) FMP-DTS is a
discriminating turbine sump BriteTM sensor; (EQ-456).
All sump sensors fixed in position at sump floor.
ALL Fill Sump Covers API Fuel Color Coded.
(3) STAGE 1 VAPOR RECOVERY BUCKETS (in Fill Sumps along with Spill Buckets)
All buckets observed clean & dry; all poppets working and with Caps.
TANK MONITORING SYSTEM: Monitor: Franklin Fueling Systems Model TS-550 evo Fuel
Management System and TS-5000 EVO (added 11/24/2014) (EQ-652 —Amended)
- Monitor appeared to be functioning properly; Sensors Status showed all OK.

- Sensor Status Printout was reviewed to ensure that the sensors properly configured to the
Fuel System; Sensor Status printout in FIRST Attachments.
(4) TANK INTERSTITIAL SENSORS: Franklin Fueling Systems model FMP-HIS-XL, Hydrostatic
Brite sensors (EQ-652).
- Sensors observed positioned in 4" of brine, up -right and in contact with the top of the tank.
- Interstitial Sensor manhole electric -box covers found tight
(5) INVENTORY PROBES (in the STP Sumps): All wiring and caps appear properly installed.
VENT RACK: (2) vent pipes and (1) diesel vent pipe appear properly installed; gasoline vents with
pressure/vacuum caps.
EMERGENCY STOP TEST: Three Emergency Shut -offs tested; each cut power to Turbines &
Dispensers.
Inspection Comments
08/30/2018
POST CONSTRUCTION TESTING: Performed by Crompco on 8/16/2018 included:
VAPOR RECOVERY /Stage II; Stage 1 Pressure Decay & Pressure Vacuum Valve, all passed.
(5) TANKS: Primaries Tightness Tested (EZY-3 Locator Plus), all passed.
(5) OVERFILLs VARIFICATION: all completed.
PRODUCT LINES: All Primaries Tightness Tested (Petro-tite Line), all passed.
LINE LEAK DETECTORS: (5) tested, all passed.
MONITOR: Monitor inspection, Sensors made to alarm; all passed.
DROP TUBE LENGTH INSPECTION: Inspected, all pass.
EMERGENCY STOP: (3) all pass.
SHEAR VALVES: operating properly; all passed.
PRODUCT LINE SECONDARY&: Pressure tested at 5 psi, all passed.
SPILL BUCKET SECONDARY&: Vacuum Tested (inches H20), passed.
FINAL HYDROSTATIC TESTS — On all Containments, all passed.
RELEASE DETECTION REQUIRED: Tanks, Dispenser Liners, Fill Sump for Spill Bucket and
Product Piping via STP Sumps all monitored by electronically by the TS 550 evo tank monitor:
examine the tank monitor for alarms, press the Test Button to assure the audible alarm and lamps
are working; must maintain one 3 Wire Sensor Status printout per month. Every six must visually
Inspected all electronically monitored equipment. All visual inspection records and tank monitor
`sensor status printouts must be maintained for a minimum of three years.
RECORDS:
- Tank Registration submitted to Tallahassee by owner; Facility photo, Site Map and Tank Diagram
entered in First page 1.
- Testing records in Attachments. Lat. and Long. coordinates to be entered into DEP Webpoint.
- Contractor and/or owner gave proper notification prior to all work.
- This Installation Inspection Report was emailed to Michael D. Zarrella/ President/ Wilson's
