Gentlemen:
Technicians from D & N Drilling & Engineering, Inc. witnessed the installation of the
Helical Piers at the above referenced address on Tuesday, September 19, 2017 starting at
9:30 AM and completing at 12:30 PM. The 100% on site observation of the installation
was completed as required per construction plans.
A review of the final pier installation logs with pier depths, locations and time of
installation can be found later in this report. There was also a review of the contractor's
qualifications including confirmation that the helical pier contractor was certified.
Based on onsite observation, review of pier installation logs and applicable contractor
data, we hereby certify that the helical pier foundation for the referenced residential site
was installed in substantial compliance with the requirements of the construction plans
and helical pier installation specifications.
It was a pleasure to assist you in this phase of your project. If you have any questions
