Hi Diana,
The balance of the fees due on the draft appIication for Trowbridge is $ ,775. In order for me to request a check I MIJ
need to have a -9 completed by whomever the check is payable to, I have several questions'
1 _ To whom do I send the blank W-9?
. Who should the check be irla' adem o?
3. Does the $2,775 cover all costs or is there anything else 1 should include'
IL can take 2 weeks or more for rye to obtain a check, so I wanted to get this part moving.

The fees on the applic8tic n are correct, just include the tot�ka. The ccncurrenq deferral is to be signed by the owner_
In the des&ptiorf please indicate you are requesting a change �n zoning from IX to ACC-5.

As we discussed I am attaching a draft rezoning application for the 11.6 acres on Trowbridge road_
believe it is complete in terms of the items required- Of course, I still need to get the owner to sign the
application once we Confirm completeness_ I assume the buyer is the one to sign the concurrenry affidavit
since it deals with post closing issues,
Could you confirm that the fee calculation is correct at $2,775. Just curious why a utilities fee is charged
since the part of the county in many miles from any public utilities. Also curious as to what the ERD fee is for.
Once we confirm completeness we will make the required 11 copies and havie 2 CD's made of all documents
with the required files named.
