2. The Fire Marshal requires 24 hour notice on all inspections.
3. The respective Building Department shall schedule all final Inspections through the Fire Marshal's Office.
4. Failed inspections require payment of fee prior to rescheduling of further inspections.
5. Penetrations through rated assemblies shall be of proper UL design. Design criteria shall be submitted
with the construction plans.
6. Fire alarm panels shall be located Indoors within a controlled environment.
7. Plans and construction are subject to corrections in the field to maintain code compliance.
An electronic version of the construction plans on a CD is required. The file
format shall be .pdf only. The CD shall contain a sheet index file.
Files shall be separated by discipline. Files names shall match construction
documents sheet numbers.

1. Provide duct smoke detector in AHU #2 (AHJ ) Audible and visual notification required attest switch.

morning. We have reviewed the shielding plan for the office of Dr. Pelestrant atthe
above address. The plane oubmt fed comply with the Florida Adminldmdm Code
(FA.C.) as long se the following cotteitions are met
a. The x-ray unit Is less then or equal to a 125 KVp unit cited.
b. The unit 19 Installed with the tube Us parapet to the outside wall With the
upright budry mounted on the wall oiV 7 W Wale' outer wall.
