Kimley-Horn and Associates, Inc. has completed a post construction visual observation of the above
referenced project at an existing rooftop based Wireless installation. The scope of work involved the
removal of (1) cabinet, the relocation of (1) cabinet, the replacement of (1) cabinet and the installation
of new decking on the equipment platform. Based on information and photographs provided, I hereby
state that the equipment affected by this project has been installed per manufacturer's specifications
and permitted plans.
This evaluation also makes use of digital photos of the as -built conditions taken after construction
To avoid any misunderstanding, I hereby state that, to the best of my knowledge, belief and
professional judgment, this report represents an accurate appraisal of T-Mobile's equipment based
upon careful evaluation of observed conditions to the extent reasonably possible.
The work has been reviewed and the upgrade has been completed in accordance with the 5"'
Edition (2014) FBC and the permitted plans.
