Interior Floor ASME V Testing as per ASTM.
Good afternoon Rick,
The cut in the structural concrete floor, which appears to be [ in photos ] approx.. 5-6" in width
1.5-2" in depth has been very concerning to all of us and we have asked that Mr. Goode rectify this issue today via a
'letter from a structural S.I. [ Special Inspector J stating that the floor is safe and will not cave in upon itself and not hurt
any individuals below it. I thought that you were certified to generate such letter and sign, date and seal the document
or we have asked that Mr. Goode shore this area from the unit below for safety of the adjacent unit owners until such a
letter may be generated.
Mr. Goode is also working with an engineering firm in Stuart for their expertise in these
situations and we are waiting to hear from that company.
Thank You

Carl,
I This is the lower floor of a 2 story penthouse unit .. The owner came in for a meeting and he had a floor x-ray
With him. He just called again and he is having PIET examination co, e-mail me a digitally sealed report for today that the
floor is safe and will not collapse. He is also putting Special Forces on notification for shoring this afternoon if we do not
receive any documentation. He is going to be working with Mathers Engineering as well today.
I have spoken with the property manager Tammy and informed her of the urgency of this issue. She stated that
the board or the owners of the unit below will not allow any shoring to happen so I have asked her to send me an e-mail
stating such.
The owner has done this himself, however will be hiring a general contractor for permitting. Atlantic Concrete
has cut the floor

I have forwarded this to my architect top make the additions. I have
attached the Nofice of Commencement, do you need the original or is this
sufficient?
I had discussed the HVAC issue with my HVAC contractor and he said
all that he was doing was to repair the ac, no changes to airflow. Is it
necessary to have load calculations? I will have him submit a sub
agreement.
