Dear Contractor,
Welcome to the Island Club. We ask Vou to please conform to our rules:
1. Be a licensed contractor.
2. Please present your current certificate of insurance.
3. You MUST park in the designated contractor parking area --- THE WEST WALL
4. Unload by the South entrance of the building, use the freight elevator ONLY, then park in
the designated area.

ON WEEKENDS
6. On your arrival, you must sign in at the office each day.
7. If you need keys, they must be signed for and returned by the end of each work day
(6:0013M). We MUST have the owner's Permission in writing, e-mail or by fax, if they are
not in their residence.
S. The PROPER CORK MUST BE INSTALLED PRIOR TO LAYING ANY FLOORING.
9. Your work area must be left clean, continually and thoroughly. The catwalks, elevators
and vestibule area must also be left clean.
10. You must sign this form before unloading or performing any work.
11. You will be charged for any and all damage you create.
12. You must provide your own tools, equipment, vacuums, mops, etc.
13. Construction debris is not to be placed in the Island Club dumpsters. You must provide for
proper disposal.
