This letter is to certify that the permitted sanitary sewer work, including the lift station
installation, gravity sewer system, and all other associated items including the proper
removal of the existing septic tanks and drain fields, included in the project plans has
been completed in substantial conformance with the design plans and specifications.
Determination to certify is based upon on -site observation of construction conducted by
my project representative under my direct supervision for the purpose of determining if
the work proceeded in compliance with the plans and specifications, and review of test
results and as -built surveys provided by the contractor.

Dear Mr. Stepalavich,
This letter is to certify that all required utility inspections to include the force main
installation directional bore, for the above -referenced project have been completed in
substantial conformance with the approved design plans and specifications.
Determination to certify is based upon review of the as -built surveys, density reports, and
testing as well as daily test reports provided by the PSLUSD. Determination to certify is
also based upon on -site observation of construction conducted by myself, or my
representative, for the purposes of determining if the work proceeded in compliance with
the plans and specifications.
