This application can be submitted to St. Lucie County Building and Zoning, 230D Virginia Avenue, Fort Pierce,
FL 34982. All permit applications must be filled out completely before submission. No applications will be
accepted for processing after 4:30 P.M. For assistance in completing this application, please call (561)
462-1553, during regular office hours (8:00 AM - 5:00 PM), Monday through Friday.

Subject: Re -roofing Structural Inspection for Building Permit Application
I have reviewed the re -roofing of the aforementioned business establishment mono -
slope roof (2:12 pitch) to assess its compliance with the current Florida Building Code
(FBC).
The existing trusses & plywood sheathing needed a re -roofing due to hurricane
damage. The fastening of the plywood sheathing consists of re -nailing 4" o.c. at the
perimeter and 6" o.c. in field using 8d ring -shank nails.
JM App. Base material (ASTM D 5147 APP modified asphalt, fiberglass -reinforced,
smooth surface base sheet) is fasten to the mono slope deck using Tru-Fast Metal
plates and fasteners at a 4" side lap, 12" o.c. and two rows staggered in the center of
the sheet 18" o.c. to achieve a maximum design pressure of -45 psf.
The top layer roofing material is APPeX 4.5M (ASTM D 6222, type I grade-G) APP
modified asphalt, Polyester -reinforced, mineral -surfaced membrane. The seal at the
parapet is achieved using turn bar attachment components.
