observations during our site visits and to the best of my knowledge, belief and
professional judgment, those portions of the project approved for special inspections
for concrete restoration work, meet the intent of the Florida Building Code 2014 and
are in substantial conformance with the project's specifications. All site visit reports
were submitted to your office throughout the construction period.

This is to confirm I will be performing special inspections for the duration of the
above -referenced project. The following inspections will be required:
1. Initial site visit to determine the areas to be repaired
2. Observation of areas to be repaired after removal of deteriorated concrete and
exposing rebar.
3. Observation of all cleaned surfaces and cleaned steel rebar prior to coating.
4. Observe all surfaces after coated with rust inhibitors and bonding agent.
5. Observe repaired areas after patching
6. Observe areas after the sealant is applied.
All inspections shall be scheduled by the contractor with minimum of one work day
notice.

and forms were installed. Observed work was found to be
acceptable and ready for pouring concrete.
