WORK INSPECTED:
Balconies at Units 201, 301 & 401 north east balconies.
OBSERVATIONS:
The walls have been sheathed with new 5/8" densglass with screws at 4" oc at edges and 12" oc in the field.
Plans require 4" at edges and field.
There is Dryvit backstop NT and grid tape at all densglass joints, at corners over screws.
The densglass runs past all the floor slab joints, except at the 2"' floor the sheathing stops at the top of the slab because
the slab at the parking garage extends further beyond past the face of the wall.
RECOMMENDATIONS:
Provide additional screws at the densglass sheathing to the metal framing to maintain 4" on maximum at all areas. Seal
the screw heads with Dryvit Backstop NT.
We need to seal the horizontal wall sheathing joint in the densglass at the 20d floor slab. The slab needs to be built out
flush with the densglass and the joint sealed with Grid tape and Backstop NT.
The window and door perimeters are also to be sealed. Thejoint or gap at the ends of concrete curbs and the metal
studs at the sliding glass door thresholds are to be filled with non -shrink grout and the exteriorjoint sealed.
I have arranged for Chris Brennan, a field rep for Dryvit with Allied Building Products, to visit the job on Monday
8/13/18 at 10 am to address some of these issues.

WORK INSPECTED:
Units 301, 401 & 501 — north east balconies — repairs are complete and ready for painting.
Units 601-901 — north east balconies.
OBSERVATIONS:
• Investigative work in progress to determine the level of deterioration or corrosion in the exterior metal stud
walls.
• In all cases the metal stud components at the ends of the walls at the sliding glass doors are corroded.
• Unit 601 — The corrosion is in the worst condition. The corrosion in the metal framing extends under the
window and the track and studs need replacement. The exterior wall under the window needs removal and
replacement. The window may remain in place, but it will require a new track and studs under the window.
The new studs can be placed next to the existing studs and clipped with an L3x3x16 ga x 4" clip with (3) #10
screws and (2)'/4" dia x 1" tapcons through the track into the concrete.
• Units 701-901— The ends of the walls at the sliding doors are corroded and some extends to the window.
• The tracks do not need replacement but will require wire brush cleaning and coating with corrosion inhibitor.
The new studs (where necessary) can be attached to the existing and clipped with an angle to the concrete slab
as mentioned above.
• Documents for units 601-901 have been submitted for building permit revision.

OBSERVATIONS:
• The EIFS is in progress.
• The Densglass is in place, joints and screw heads were taped and coated. The entire surface was then coated
with the Backstop NT air and moisture barrier.
• The gaps at the end of the concrete curbs under the sliding glass doors were filled with cement grout.
• Backer rods and sealant was placed between the joints at the door and window frames and walls.
• The finish coat was ground off at the joint in the insulation and the new mesh tape and backstop overlapped.
• The AquaFlash tape and liquid was placed at the door and window perimeters. More flashing needs to be
added. See recommendations below.
RECOMMENDATIONS:
Provide AquaFlash tape over and onto the window and door frames to cover across the joints between the frame and
wall. Extend the AquaFlash tape at the bottom of the wall next to the door outward to pass the adjacent joint. Properly
lap all flashing and tape at door and window perimeters. The upper layers should be lapped over and above the lower
layers.

Balconies at Unit 301 & 401 north east balconies
OBSERVATIONS:
Units 301 & 401
The concrete balcony repairs are completed and repairs are in progress at the exterior walls at the sliding glass doors.
The SGD and corroded metal studs at the exterior wall between the doorjambs and windows have been removed.
A section of the 6" metal tracks at the top and bottom near the doorjambs will need to be replaced and spliced.
This confirms that the walls have been leaking at the doorjambs due to inadequate waterproofing resulting in the
deterioration of the metal stud framing.
RECOMMENDATIONS:
Replace the exterior EIFS wall system, metal studs and tracks as plan.
The new waterproofing shall be lapped with the existing at all joints.
Clean and coat the minor corrosion at all the exposed metal studs and tracks that are to remain.
Verify if the new doorjamb can be constructed providing adequate room for connection of the existing guardrail so that
the SGD can be re -installed in the same location without having to modify the opposite doorjamb.

WORK INSPECTED:
Balconies at Units 301, 401 & 501 east balconies
OBSERVATIONS:
Unit 301
Demolition is complete and there are (10) PT cables that are affected at the north east corner. One of those cables still
requires a splice with a longer cable and PVC splice protector.
All the reinforcing bars are cleaned and coated, additional #5 bar is to be added where the existing bar was cut to
facilitate the lock off in the middle of the slab.
This will require the concrete pour back to be sequenced so that several cables can also be restressed in sequence.
Concrete pour is approved provided the one cable that still needs splicing can be placed after the first pour.
Temporary weather wall is in place and the sliding glass door has been removed and demolition to repair the exterior
walls is in progress.
Unit 401
Work in progress, but unit 501 is being shored to this floor.
Unit 501
Demolition is complete at the north east corner.
One of the existing post tension cables was found broken due corrosion of all the strands.
A total of (6) PT cables had repaired. (5) had be locked off destressed and spliced and the (1) broken cable spliced.
No existing reinforcing bars required replacement and the bars were cleaned and coated with corrosion inhibitor.
Concrete approved for placement.
RECOMMENDATIONS
Although a temporary wall was placed inside the units where the sliding glass doors are removed, the Contractor is to
have materials on site ready to enclose the door opening in case poor weather could possibly blow into the units from
the north east.

Balconies at Unit 301 & 401 north east balconies
OBSERVATIONS:
Units 301 & 401
The exterior walls have been reconstructed with new metal studs, sheathing and EIFS and the sliding glass doors
reinstalled in the original location. There is no interior drywall as yet which allows for inspection.
The new metal studs are 6" but are not 18 gage structural studs.
There is no Sto Gold Coat air and moisture barrier on the desglass behind the insulation system.
RECOMMENDATIONS:
Remove and replace the exterior EIFS wall system wich includes densglass joint tape and the Sto Gold air and moisture
barrier prior to installing the insulation system.
Install the correct 6"-18 ga structural studs which should have a minimum 1-5/8" flange width
The horizontal joints the exterior wall located at the top and bottom of the slab lines should return to the slab edges
around the building corners. The joints at the exterior walls at the slabs top & bottom at the balconies are to be sealed
with a chamfer strip of urethane sealent to match.

Item(s) Tested: Fall Protection Anchors
Building: 4400 North Highway AlA, Fort Pierce, FL 34949
DHG Job No.: 2018-212, Task 01
Dear Mr. Barber:
As requested, on April 13, 2018, load testing of the seventeen (17) anchors located on the roof of
the referenced building was conducted. During testing, the system was observed for
unusual/excessive deflection or movement. The fall protection anchors were loaded to a specified
proof test load below. Please reference the drawing Sheet 1.0 of the anchor layout for additional
information on anchor location.
In order for the proof load testing to demonstrate compliance with OSHA specifications, the fall
protection anchor shall comply with the following;
"1926.502(d)(15)(i) as part of a complete personal fall arrest system which maintains a
safety factor of at least two; and "
"1926.502(d)(16)(if) limit maximum arresting force on an.'emp' hyee to 1,800 pounds (8
Kly) when used with a body harness; "
The test loads were applied, sustained, and monitored for a minimum 2,500 lbs to 2,700 lbs
horizontal force for a minimum three (3) minute load duration. The test load represents an
approximate 150% proof load based upon the maximum arresting force of 1,800 lbs. The specified
test load is based on the anchor being properly designed to adequately support the maximum fall
arresting force of an employee with a safety factor of 2:1 against ultimate capacity. Deflection was
observed, along with visual observation of any permanent deformation and/or damage done to the
anchor.
All anchors achieved the 2,500 lbs to 2,700 lbs proof test when the loop was tested in a direct
tension application. No permanent deformation was visually observed and the specified load value
was held for a minimum three (3) minute load duration. A picture of the fall protection anchor test
setup can be seen in Figure 1.
Pigure 1 — Fall Protection Anchor Test Setup — T.v.. "°.' —•
CONCLUSION
Based upon the load testing, visual observations, and our engineering experience with similar type
systems, DH Glabe & Associates have determined the fall protection anchors at the roof
locations outlined in this report are acceptable for use. If no significant changes or
modifications are performed, DH Glabe & Associates should be contacted to re -observe the system
prior to April 2019.
Limitations: DH Glabe & Associates cannot assume liability from any damage associated with unknown conditions

West balconies 4N 5N, 6N, 7N, & 8N.
OBSERVATIONS:
Units 4N, 5N & 8N:
• There were 23 PT affected cables at these three units which were locked off, destressed and spliced with new
cable ends.
• Unit 4N 8 PT cables
• Unit 5N 6 PT cables
• Unit 8N 9 PT cables
• All reinforcing bars at these units have been cleaned, coated and replaced where needed and concrete can be
repented at these balconies.
• The EIFS walls where the PT cables had to be locked off were damaged and will require repairs.
Units 6N & 7N:
• The PT cable ends which were exposed but are not being replaced are being cut back and the anchor plates
coated with corrosion inhibitor. Work is still in progress to clean and repair the corroded reinforcing bars
along the balcony edges and cut back to achieve the minimum concrete coverage where necessary.
Unit 3N
• We observed some discoloration due to efflorescence at the underside close to the edges at this balcony at 3
locations. This is most likely caused by moisture intrusion through the cracks in the concrete slab under the
tiles or along the edge. We have decided to monitor the slab at this time.
RECOMMENDATIONS:
• Repour the balconies where the PT cables have been replaced.
• Continue the edge repairs on the remaining balconies.

Units 601(6S) through 901 (9S) exterior wall repairs
OBSERVATIONS:
• The exterior metal stud framing and EIFS system have been repaired as per plan.
o Water tested the repaired work at the SGD and walls. No water intrusion was found.
• Spalling was found along slab edge at 7S which will require repairs. Some of these areas appear to have been
previously repaired.
RECOMMENDATIONS:
Attach the metal studs to the adjacent jamb studs or full height studs with 2 #10 screws at 4" oc vertically at
the top and bottom of the window jambs at the repaired areas.
There is evidence of water intrusion at the exterior wall and around the window at the second floor. We
recommend the base board and some drywall be removed to assess the damages to determine if the wall needs

West balconiesST, N, 6N, 7N, & 8N.
OBSERVATIONS:
Demolition is in progress at the North balconies' west elevation at the above units.
Additional corrosion and deterioration of the balcony edges are being uncovered as the demolition progresses.
We observed many of the PT cable ends were not capped, were left too long, and extended to the surface of the slab and
are now corroded. The reinforcing bars along the slab edges also had inadequate coverage and were corroded.
1;jl i LIcony edge requires repair. There are 3 PT cables that need to be repaired to include being locked off,
destressed, spliced and re -stressed. Due to the geometry of the balcony, this may affect up to 10 PT cables
since they may be overlapping.
5N 4 PT cables require repairs. This will affect more cables..
6N: This balcony had ceramic file installed, which is being removed. The tile was found to be loose and unbonded
to the concrete slab, and was tented in some areas due to thermal movement. The thickness of the tile and
thinset appeared higher than the interior slab which would trap -moisture against the exterior metal stud infill
walls. The affected PT anchors may be repaired and replacement is not required. The cable ends need to be
cut back and capped with a greased end cap.
7N: 4 PT anchors are exposed and require repair, but replacement is not required as above.
There were 2 corroded 1" diameter wedge bolts found the middle of the slab which were removed. These
anchors appeared to have been installed during a renovation and were left in the slab.
8N: 4 PT cables require repairs. This will affect more cables.
RECOMMENDATIONS:
• Continue all concrete, reinforcing and PT cable repairs as per plan.
• Coordinate the cable repairs with the PT specialist.

WORK INSPECTED:
Units 301, 401 & 501— north east balconies — repairs are complete and ready for painting
Units 601-901—north east balconies.
OBSERVATIONS:
• Investigative work in progress to determine the level of deterioration or corrosion in the exterior metal stud
walls.
• In all cases the metal stud components at the ends of the walls at the sliding glass doors are corroded.
• Unit 601— The corrosion is in the worst condition. The corrosion in the metal framing extends under the
window and the track and studs need replacement. The exterior wall under the window needs removal and
replacement. The window may remain in place, but it will require a new track and studs under the window.
The new studs can be placed next to the existing studs and clipped with an L3x3xl6 ga x 4" clip with (3) #10
screws and (2) '''A" din x 1" tapcons through the track into the concrete.
• Units 701-901 — The ends of the walls at the sliding doors are corroded and some extends to the window.

Balconies at Units 201, 301 & 401 north east balconies.
OBSERVATIONS:
The walls have been sheathed with new 5/8" densglass with screws at 4" oc at edges and 12" oc in the field
Plans require 4" at edges and field.
There is Dryvit backstop NT and grid tape at all densglass joints, corners at over screws.
The densglass runs past all the floor slabjoints, except at the 2"d floor the sheathing stops at the top of the slab because
the slab at the parking garage extends further beyond past the face of the wall.
RECOMMENDATIONS:
Provide additional screws at the densglass sheathing to the metal framing to maintain 4" on maximum at all areas. Seal
the screws heads with Dryvit Backstop NT.
We need to seal the horizontal wall sheathing joint in the densglass at the 2"d floor slab. The slab needs to be built out
flush with the densglass and the joint sealed with Grid tape and Backstop NT.
The window and door perimeters are also to be sealed. The joint or gap at the ends of concrete curbs and the metal
studs at the sliding glass door thresholds are to be filled with non shrink grout and the exterior joint sealed.
I have arranged for Chris Brennan, a field rep for Dryvit with Allied Building Products to visit the job on Monday

Remove and replace the exterior EIFS wall system with includes densglass joint tape and the Sto Gold air and moisture
barrier prior to installing the insulation system.

WORK INSPECTED:
Balconies at Unit 301 & 401 north east balconies
OBSERVATIONS:
Units 301 & 401
The concrete balcony repairs are completed and repairs are in progress at the exterior walls at the sliding glass doors.
The SGD and corroded metal studs at the exterior wall between the door jambs and windows have been removed.
A section of the 6" metal tracks at the top and bottom near the doorjambs will need to be replaced and spliced.
This confirms that the walls have been leaking at the doorjambs due to inadequate waterproofing resulting in the
deterioration of the metal stud framing.

Balconies at Units 301, 401 & 501 east balconies
OBSERVATIONS:
Unit 301
The balcony edge has been poured at the PT acnhors and all but one cable has been re -tensioned and lock offs removed.
One cable still requires a splice with a longer cable, PVC splice protector placed and to be restressed.
Provide a new 95 bar to replace the bar that was cut out.
All the other block outs may be poured with the exception of this area.
Unit 401
Work in progress
Unit 501
The balcony edge has been poured and all the cables re -tensioned and lock offs removed.
There is an existing reinforcing bar in the slab running east to west that has minimal concrete cover but this existing bar
cannot be repositioned. Clean the bar and coat with corrosion inhibitor and build up the slab thickness as high as much
as possible to maintain positive drainage.
RECOMMENDATIONS
PT specialist to provide a calibration certificate for the ram being used to tension the cables.
PT cables to be cut shorter and greased end caps placed prior to grouting cable pockets when complete.
Sikaflex cementitious water proof coating to eb placed on all slab surfaces. Some balconies have tile and others have a
knock down concrete finish.

Concrete pour is approved provided the one cable that still needs splicing can be placed after the fast pour.
Temporary weather wall is in place and the sliding glass door has been removed and demolition to repair the exterior
walls is in progress.
Unit 401
Work in progress, but unit 501 is being shored to this floor.
Unit 501
Demolition is complete at the north east comer.

WORK INSPECTED:
Balconies at Units 301, 401 & 501 east balconies
OBSERVATIONS:
Demolition of the areas to be repaired is in progress. Post tension (PT) anchors and reinforcing bars at these areas are
exposed to determine the extent of repairs and PT cables that will be affected.
Swing stage fall protection anchors at the roof were load tested and certified by DH Glabe & Associates. (copy is
attached)
Unit 301
• The PT anchor was not encapsulated with an end cap and the cable was exposed
• Two PT cables are to be locked of and spliced
• Additional crack at the underside of the slab at the north east comer which will require repair
Unit 401
• Reinforcing bars at slab edge are corded due to being too close to surface
• PT cable ends did not have end caps and cables were not cut short enough
• Three PT cables to be locked off and spliced
Unit 501
• PT cables have end caps
• Reinforcing bars too close to surface and corroded
RECOMMENDATIONS
Continue the demolition at the areas to be repaired and coordinate with General Building Services (PT Technician) to
lock off affected PT cables.
Some shutter tracks will require removal to facilitate the concrete balcony edge repairs.
Ceramic floor tile should be removed only where necessary along the grout joints.
Provide temporary cover over all openings in floor slab and barricade all areas where the guard rail post is not secure.
