AEL Ila •.No.cP at&t September 22, 2017 . SCANNED Dennis Demarco St. Lucie CoBY un MasTec Network Solutions, LLC tY E N G I N E E R I N G G R O U P 6100 Broken Sound Parkway, Suite 6 Boca Raton, FL 33487 Subject: Structural Analysis - Antenna Mount Carrier Designation: Midway Rd [FA# 10048909] F LE © PAT&T:ry APX Project: 17-0107.95 Site Information: 5690 Environment Drive Fort Pierce, Saint Lucie County, FL 34981 Latitude 27.3795910 N, Longitude 80.3821000 W Existing Antenna Mount (Worst Case) Dear Mr. Demarco: APX Engineering Group, LLC (APX) is pleased to submit this Structural Analysis to determine whether the subject worst -case existing antenna mount is able to support certain proposed loads. It is our understanding that AT&T, who retained MasTec Network Solutions (MNS), desires to install new telecommunication equipment on the subject mount. This analysis was based on the supporting information listed in Table 2. Our services were performed in accordance with the terms and conditions of the master service agreement between APX and MNS. This report summarizes the results of our findings. The purpose of this analysis is to determine the suitability of the aforementioned structure to support the loading indicated in the attached calculations. This analysis has been performed in accordance with the Florida Building Code based upon an ultimate 3-second gust wind speed of 158 mph using RISA-3D structural software. Based on our analysis, subject to the assumptions, it is our opinion that the subject structure will not support the proposed loading. We recommend replacing all existing mounts with Connect -IT Wireless #PVFM12-3-3-96-3 heavy-duty v-frame mounts. Existing Members: Fail at 122.8%, Existing Connections: Pass at 20.7% This report has been prepared for the purpose of providing a structural evaluation of the subject structure for the loading conditions indicated. It is intended for the exclusive use of AT&T and MNS. The information, assumptions, and recommendations contained in this report should not be used by others for any purpose without express written authorization from APX. We appreciate the opportunity to provide our professional services to you and look forward to continuing our relationship. If we can be of any further assistance, please do not hesitate to contact us. _.AIIMMisp— Sincerely, APX Engineering Group, LLC Michael A. Phillips, P.E. Information in the supporting documentation that could not be verified by APX personnel is assumed to be correct. 2. All structural components are assumed to be in adequate condition to carry their full design capacity. 3. The configuration of equipmentlappurtenances is assumed to be as specified in this report. 4. Emergency/essential communication equipment will not be supported by this assembly. If any of the above assumptions are not valid or have been made in error, these calculations may be affected, and APX should be allowed to review any new information to determine its effect on the structural integrity of the subject structure. Recommendations 1. Replace all existing mounts with new Connect -IT Wireless #PVFM12-3-3-96-3 heavy-duty v-frame mount assemblies.
