items you require based on the Rep rt-FL-Review-Com me nts-Non com p lia nt form you attached.
Documents missing -Audrey Humphrey:
1. The electrical sub agreement is attached (813812-SLC_Apps_exec).
2. A recorded Notice of Commencement is attached.
3. Phone number for the tenant - 561-922-1230. Contact name Adam Ehrlich
Zoning Review:
1. Lease agreement, attached (2).
2. Security Bond information form, attached.
3. Copy of the original bond provided on 1/23/18.
Should you need anything else, please let me know. Otherwise, I hope this satisfies all outstanding issues and we can
pick-up our permit.

In response to your phone call this morning. At 1/16/18 we have sent an update of the missing documents. At 1/23/18
we have received the copy of the bond. The other requested documents were not attached. Please send a copy of the
lease agreement and the security bond form so we can release the permit to the Permitting Dep.
