The purpose of these inspections will be to determine if the installation of the antenna equipment and antenna platform mount is in substantial accordance with the approved construction drawings prepared by APX dated November 17, 2017. Such special inspections will be performed in accordance with Section 110.10.1.1 of the Florida Building Code and will include, but not be limited to, the following: 1. Inspect anchoring, spacing and mounting of the proposed antennas to the existing tower structure. 2. Inspect mounting of proposed remote radio units (RRLIs) and proposed peripheral equipment to proposed antenna mounts and anchoring and spacing of proposed antenna mounts. 3. Inspect the installation of the proposed tower retrofit and antenna mount bracing. Copies of the inspections shall be sent to the Building Department on a weekly basis, and the final letter of completion will be provided at the completion of the final inspection. This letter has been prepared for the purpose of fulfilling the requirements of the Saint Lucie County with respect to the specific installation described. It is intended for the exclusive use of the Saint Lucie County and AT&T. The information, assumptions and recommendations contained in this letter should not be used by others for any purpose without express written authorization from APX. Inspection Date: October 15, 2019 Saint Lucie County Planning & Development Services Building & Code Regulation Division 2300 Virginia Avenue Fort Pierce, FL 34982 a pA E N G IN E E R ING G R O UP Subject: Special Inspection Report 1 of 1 (Final) [3C/4C/5C] Permit: AT&T Site: �8>D FP12 (F076) Site Address: 3301 Orange Avenue, Fort Pierce, FL 34947 APX Project: 17-0107.37 Dear Building Official: APX Engineering Group, LLC (APX) performed a review of construction and close-out documentation of the AT&T telecommunication facility located at the above -referenced site. The purpose of these observations was to inspect the following in accordance with the Florida Building Code, 5v' Edition (2014) and the National Electric Code (NEC), 2011 Edition: 1. Removal of nine existing antennas. 2. Installation of nine proposed antennas. 3. Installation of six proposed Remote Radio Units (RRUs). 4. Installation of one proposed surge suppressors. 5. Installation of one proposed WCS filter. 6. Installation of three relocated stiff -arm braces. 7. Installation of proposed power cables. 8. Installation of proposed fiber cables. 9. Installation of proposed grounding system. OCT 2 2 2019 Permitting Department St. Lucie County, FL The installation of the above -mentioned items was visually inspected and compared with the approved construction drawings by APX Engineering Group, LLC dated March 28, 2018. Based on these observations and to the best of my knowledge and belief, the above -described construction of all structural load -bearing components complies with the permitted documents.
