May 23, 2017 0 S ear DEC 19 2011 L a x Dennis Demarco MasTec Network Solutions, LLC E N G IN E E R ING .G R O UP 6100 Broken Sound Parkway, Suite 6By' """""""""""' Boca Raton, FL 33487 SCANNED BY Subject: Structural Analysis - Antenna Mounts [3C] St. Lucie County Carrier Designation: AT&17-01 .37 [FA# 10070076] OPY APX Project: 17-0107.37LE C Site Information: 3301 Orange Avenue Fort Pierce, Saint Lucie County, FL 34947 Latitude 27.4461800 N, Longitude 80.3595720 W Existing Antenna Mounts (Worst -Case) Dear Mr. Demarco: APX Engineering Group, LLC (APX) is pleased to submit this Structural Analysis to determine if the subject existing antenna mounts are able to support certain worst -case proposed loads. It is our understanding that AT&T, who has retained the services of MasTec Network Solutions, LLC (MNS), desires to install new telecommunication equipment on the subject mounts. This analysis was based on the supporting information listed in Table 2. Our services were performed in accordance with the terms and conditions of the master service agreement between APX and MNS. This report summarizes the results of our findings. The purpose of this analysis is to determine the suitability of the aforementioned structures to support the loading indicated in the attached calculations. This analysis has been performed in accordance with the Florida Building Code based upon an ultimate 3-second gust wind speed of 158 mph using RISA-3D structural software. Based on our analysis, subject to the assumptions and recommendations noted, it is our opinion that the subject structures will support the proposed loading. Existing Members: Pass at 88.3%, Existing Connections: Pass at 61.4% This report has been prepared for the purpose of -providing a structural evaluation of the subject structures for the loading conditions indicated. It is intended for the exclusive use of AT&T and MNS. The information, assumptions, and recommendations contained in this report should not be used by others for any purpose without express written authorization from APX. We appreciate the opportunity to provide our professional services to you and look forward to continuing our relationship. If we can be of any further assistance, please do not hesitate to contact us.-asaaea9Fa; ,• Sincerely, APX Engineering Group, LLC Gertha Wesh Michael A. Phillips, P.E. Category 1 Crest Height Zero Feet Analysis Assumptions and Limitations 1. Information in the supporting documentation that could not be verified by APX personnel is assumed to be correct. 2. All components are assumed to be in adequate condition to carry their full design capacity. 3. The configuration of equipmentlappurtenances is assumed to be as specified in this report. 4. Emergency/essential communication equipment will not be supported by this assembly. If any of the above assumptions are not valid or have been made in error, these calculations may be affected, and APX should be allowed to review any new information to determine its effect on the structural integrity of the subject structure. Recommendations 1. Relocate the stiff -arm brace connection point at the mount to within one foot of the top mount face elevation. 2. Check all bolts for proper tightening in accordance with AISC "Snug -Tight" method prior to installation of proper equipment. 3. Replace any rusted/corroded hardware with new galvanized hardware.
