Please be advised that qualified personnel of Cook & Menard Architecture, Inc. performed an onsite
inspection at 2307 Elizabeth Avenue regarding the above referenced construction. We find that the
construction meets with our plan specified requirements as well as the requirements of the 2004 Florida
Building Code with amendments for 130 MPH, 3 second gusts wind zone.
