alteration. The information to be provided with this application includes:

property where the building activity is taking place.
2. Parcel ID Number Indicate the tax identification number of the property
where the building activity is taking place.
3. Description of Project or Work Activity Fully describe the activity to take place.
4. Owner Information Indicate the name and address of the owner of the property

5. Contractor Information Indicate the State of Florida registration number (if
applicable), St. Lucie County contractor license number
and the name of the business doing the work.
6. Value of Construction
- Indicate the total value of the work to take place. Total cost
ofconstructionincludes all current`retail material and labor
costs associated with the building/construction activity.
Construction value is used to determine the permit fee. St.
Lucie County reserves the right to question and/or modify
the indicated value of construction if it is demonstrated that
the submitted figures are not consistent with similar -types

This permit referenced above is for the Sands on the Ocean Condominium Association, for the addition
of speakers to residential sleeping units. Originally the permit was attained by Mr. Allen Fonkin of
Qwest Systems Inc. however Mr. Fonkin has suddenly passed away. Our firm, Comprehensive Fire
Alarms and Security Systems, Inc., has contracted with the Association to complete the installation
which Qwest Systems began.
As discussed during our visit with your department, we have asked our Client to forward to you a letter
stating their desire to change contractors. Additionally, we have completed the "Specialty Contractor"
form you provided as requested- The form has been forwarded to the Association for the completion of
their affidavit section and will be forwarded to your department shortly.
Upon review of the approved drawings, existing conditions and feasibility of installation it is our desire
to modify the splice juncture locations from the terminal cabinet (as submitted by Qwest) to a junction
box in closer proximity. It is our intention to complete the project in accordance with the applicable
codes, for the sake of distinction we will be calling for a partial electrical rough inspection to establish
the separation of workmanship between the two companies.

Description
Lowell's Speaker Series ULS is engineered to meet
Underwriters Laboratories Standard 1480 5° edition. Similar to
the previous UL listed ULS Series, the updated and more pow-
erful 8" speaker assemblies provide dear and concise signal
reproduction for indoor alarm evacuation and/or voice commu-
nication in fire alarn/emergency warning systems and for
indoor general voice communication in non -fire alarm signaling
applications. They are also designed to meet the applicable
requirements of NFPA72A
Each assembly includes an 8' single voice coil speaker with a
dual cone, wired dual voltage 70/25V transformer with multi -Tap
power output lugs, parallel input leads, DC blocking capacitor,
and selection of round or square grille.
The ULS Series 15-watt speaker delivers excellent sensitivity
per the Underwriters Laboratories' measurement standard of 2
watts at 10 feet. This recognized standard assures building
owners and occupants that emergency communications will be
heard and understood. Recommended applications include
high-rise hotels, apartment complexes, schools, and pub-
lictretail establishments of all kinds.
The Lowell Series ULS fire protective signaling speakers must
be used with compatible control equipment and must be
matched with companion Lowell backboxes (see page 2).

8" Speaker Assemblies for Fire Protective Signaling
A & E Specifications
The fire -protective speaker assembly shall be Lowell Series
ULS. The speaker shall be an 8" single voice coil, with a dual
cane assembly. It shall have a factory mounted dual voltage
70125V transformer with multi -tap (1/4, 112, 1, 2, 5W) power out-
put leads and parallel input lugs. The speaker shall have a 15-
watt power rating and a sound pressure level of when

9. GENERAL INSTALLATION
9.1 LOUDSPEAKERS
The contractor shall ensure that the installation of speakers and horns complies with AS
2220.1 and AS 2220.2 and will provide the sound coverage required by AS 2220. The
quantity and power rating of the loudspeakers allowed for in each evacuation zone shall be
submitted at the time of tender.
The loudspeakers shall comply with the following requirements.
Ceiling mounted speakers shall:
• Be 100 mm (minimum) diameter.
• Have a frequency response of at least 100 Hz to 12 kHz.
• Have a minimum power rating of 5 watts r.m.s.
• Be fitted with a tapped transformer to enable various power ratings ranging from
approximately 0.25 watts r.m.s. to 5 watts r.m.s. to be drawn from a 100V line by a
simple re -arrangement of tapping. Speaker transformers shall be attached to the
speakers and shall be fitted with approved terminals for termination of cables.
• Have adequate acoustic baffling fitted behind to prevent the travel of sound to
other areas.
• Be complete with a perforated ceiling grille, finished to the Contract Administrator's
requirements.
• Be of a design that is suitable for the environment in which they are installed.
Reflex horn loudspeakers shall:
Have a minimum power rating of 10 watts r.m.s.
• Be fitted with a tapped transformerto enable various power ratings ranging from
approximately 1 watt r.m.s. to 10 watts r.m.s. to be drawn from a 100V line. The
transformers shall be enclosed either as an integral part of the horn or shall be
mounted immediately adjacent to the horn within a dust proof enclosure.
• Be of a design that is suitable for the environment in which they are installed.
9.2 VISUAL ALARMS
In the high noise level areas shown on the drawings where the audio system will not provide
adequate warning, supplementary visual strobe warning shall be provided. The quantity,
position and specifications of the visual alarms allowed for in each evacuation zone shall be
submitted at the time of tender. The contractor shall ensure that the installation of visual
alarms complies with AS 2220.1 and AS 2220.2.
9.3 WARDEN INTERCOMMUNICATION PHONES (WIPs)
Warden Intercommunication Phones (WIPs) shall be provided for every evacuation zone
and/or floor level in accordance with AS 2220. They shall be located as indicated on the
drawings. WIPs shall be wall mounted in purpose made cabinets at no greater height than
1525 mm above finished floor level. The WIPs shall be recess mounted except in carparks,
stairwells and plantrooms. The WIP telephone hand -sets shall be red in colour, SSL
approved and documented by the EWIS manufacturer as a compatible type.
