Dear Building Official:
APX Engineering Group, LLC (APX) performed observations of the FPL telecommunication equipment
shelter foundation located at the above -referenced site. The purpose of these observations was to inspect
the following in accordance with the Florida Building Code, 6t^ Edition (2017):
1. Removal of existing equipment shelter.
2. Removal of existing equipment shelter foundation.
3. Installation of foundation steel reinforcement.
4. Installation of foundation concrete.
The installation of the above -mentioned items was visually inspected and compared with the approved
construction drawings by APX Engineering Group, LLC dated October 5, 2018. Based on these
observations and to the best of my knowledge and belief, the above -described construction of all
structural load -bearing components complies with the permitted documents.

MOISTURE/DENSITY TESTING SERVICES
To Whom it may concern:
In accordance with the Florida Building Code, NOVA Engineering and Environmental LLC has
performed compaction testing at the above referenced project.
As part of compaction testing, we performed modified proctor test in accordance with ASTM D-
6398 for the material used as backfill (fill) and field density testing to verify the compaction efforts.
To the best of our knowledge compaction efforts for the foundation and building pad were
performed in accordance with the approved construction documents and passed the 95%
minimum compaction requirement as per the Florida Building Code and Geotechnical Report.
Furthermore, it is our opinion that the compacted soils meet with the required design bearing
capacity of 2,000 pounds per square foot, as detailed in APX Engineering's design drawings.

I appreciate your time to discuss the design requirements of this site this morning. As we spoke, the State of Florida DCA
approved shelter specified for this site has been designed for a 190mph wind speed to allow it to be used throughout the
state of Florida whereas the foundation has been designed for 169mph as required by the Florida Building Code for this
specific site. The shelter design wind speed is in excess of the minimum design wind speed required per FBC Figure
1609.3(2), thus is also acceptable and no changes are needed in our drawings.
