This report is based on the review of the product information for applicable Florida Building Code 2017 (6a' Edition), approval of materials and installation. The sequence of work shall be as follows: Per Lapolla Spray Polyurethane Foam - Foam Lok LPA 2500, 2800, 3000 over Steel Decks Installation (N.O.A. +1=3UO 04 08)-metal surfaces are to be primed with Therm- O-Prime according to the LAPOLLA Industries Inc and coating manufacturer's recommendations. Primer shall be thoroughly cured prior to application of form. Spray apply a minimum of 1"of 2.8 lb. density polyurethane foam over existing steel roof deck. • Coat all flashing and parapets with Lapolla elastomeric coating (25 mils) to the polyurethane at a minimum rate of 3.0 gallons per 100 sq. ft. The following materials were used in this repair (see attached manufacturers specifications): • Lapolla Spray Polyurethane Foam -Foam Lok LPA 2500, 2800, 3000 over Steel Decks Installation per N.O.A. # l@-r040A4TQ • Lapolla Industries, Inc. Therm-0-Flex TF 1000 Series Elastomeric Roof Coating Installation per N.O.A.1=6 055,-09- The above sequence is general in nature and may have to be modified as restoration work progressed during installation of the Foam Roof System over the building. This alternation is to enhance the insulation value, and enhance the moisture resistance of the existing roof system. The N.O.A. indicates the "Spray Polyurethane Foam - Foam Lok LPA 2500, 2800, 3000 over Steel Decks" Roof System has a-212.15 psf allowable wind uplift load for recover over steel deck membranes. This is greater than the calculated windload uplift component generated by a 170 mph-3 second gust, Exposure C, event on a roof pitch 0 to 7 degrees commercial roof system. Based on the Florida Building Code — Existing Building provisions, the roof repairs and alterations do not structurally alter the existing roof structural system and a requirement to upgrade it by this permit application is not warranted. Installation of the PUF roof system shall be based on the 2017 Florida Building Code (6 h Edition) Section-1U2L 18 to ensure that the PUF system Product Approval has been carried out with satisfactorily results. Information on work sequences, and materials were supplied by Whiting Construction. Mathers Engineering Corp. confirms that the materials meet the windload requirements of the Florida Building Code.
