It includes a schedule of Special Inspection Services applicable to this project as well as the name of the Special Inspector. The Special Inspector shall keep records of all inspections and shall furnish inspection reports to the Building Official, Owner, Engineer of Record and Contractor. All discrepancies shall be brought to the Contractor's attention, and if such discrepancies are not corrected, these discrepancies shall be brought to the attention of the Building Official and Structural Engineer of Record. The Special Inspector shall submit a final report to the Building Official documenting completion of all Special Inspections and corrections of any discrepancies prior to issuance of the Certificate of Occupancy. Special inspections do not relieve the Contractor of his or her responsibilities to carry out quality control inspections and testing. Pierce, Florida CONTRACTOR: Daniello & Associates PROJECT MGR: Chris Arias INSPECTED BY: Mike Lue WEATHER: Sunny WORK INSPECTED: Balcony slab edge at Unit 506. OBSERVATIONS: Balcony slab edge demo is complete. Balcony guardrails are remaining and access to balcony has been temporally blocked. The corrosion has been chased until clean bars were found. The top 94 perimeter bar required replacement and has been epoxy doweled into the existing slab at each end. The remaining existing bars required power tool cleaning and all bars have been coated with an approved corrosion inhibition. The edge has been formed and checked for concrete clearance and is approved for pour. Approximately 9 cubic feet of slab edge was repaired. OBSERVATIONS: Demolition has been completed and loose concrete has been removed at all spalled areas. Those areas areas follows: Unit 101 — Concrete spall approx. 1 square foot Units 101/102 — Loose decorative topping was removed, approx 20 square feet Unit 304 — Concrete spall approx. 14 square feet Unit 501— Concrete spall approx. 6 square feet. Wire ties shall be added to hold the top bars down from the top of slab. Unit 503 — Concrete spall approx. 4 square feet Unit 604 — No spall was found Unit 704 — Concrete spall approx. 4 square feet All reinforcing bars have some surface corrosion and require power tool cleaning and coating with an approved cossorion inhibition. The purpose of this special Inspection as required by the FBC, is to report and record the repair procedures highlighting those that are not in compliance with the Contract Documents. 2. The Special Inspector shall meet the requirements of the Board of Building Codes and Standards. The special inspector shall be qualified as a building inspector under Part )M of Chapter 468, Florida Statutes, or certified as a special inspector under Chapter 471 or 481, Florida Statues 3. The Special Inspector must solely dedicate his time to the Project and shall not be allowed to provide other services or work, such as material or concrete testing on the same project. 4. The Contractor shall cooperate with and assist, however possible the Special Inspector in performing his inspections. The Contractor shall allow the Special Inspector access to the project at all times when work is in progress. The Contractor shall review this plan and coordinate the schedule of work such that a minimum of 24 hours notice is given to the Special Inspector before the work that requires inspection is performed. 5. The Special Inspector shall be responsible to the Owner and Building Department having jurisdiction for the project. 6. The Special Inspector shall perform site visits at regular intervals to ensure that the inspections are being performed by his on -site representative in accordance with the Threshold Plan. 7. The Special Inspector shall cooperate with the Contractor and shall immediately notify the Contractor of all discrepancies and deviations from the Contract Documents, and the Contractor shall immediately maker every effort to rectify said discrepancies or deviations. 8. Upon completion of the structural repairs, a signed and sealed statement by the Special Inspector must be submitted to the building department stating that the portion of the project under his inspection responsibilities and under the Threshold Plan was constructed in substantial conformance with the Contract Documents. 9. This document is in addition to the requirements of the standard building inspections as required by the Florida Building Code and is not meant to specify all inspections required by that Code.
