After our recent on -site Inspection, we are pleased to present the following proposal for your review. SCOPE OF WORK: UPPER MAIN PARTIAL ROOF REPLACEMENT ,(Goat x25,70.4.00_ [AREAS OF REPAIR: APPROXIMATELY 1,675 COMBINED FTC] 1. Remove and replace all "wet" roofing (up to 1,675 ft?) as indicated in the Roof Moisture Survey completed and provided by Cleary Inspection Services, Inc. The areas of partial replacement will be performed in accordance with Florida Building Code FL292948-R12.(page 38 of 50) and installed as follows: • Cut out and remove the existing wet roofing down to the lightweight concrete deck. • Mechanically fasten one:ply of Permaply 28"' base sheet to the deck with 1.7° Lightweight Concrete (LWC) CR Base Fasteners. along the minimum 4" ;side laps at7" o.c. and 7" o.c: in three staggered rows in the centerof the sheet • Install one ply JM DynaWeld'" base SBS=TA modified bitumen membrane over the base sheet and tie-in to the existing roof, heat welded. • Install one ply of JM DynaWel&' Cap SBS=TA modified bitumen membrane -over the base sheet and tie-in to the existing roof, heat welded. Re -flash [7) cooling tower and/or screen wall column curbs found within the area of repair. 2. Remove and replace all pitch pans within the area of repair with self -leveling sealant followed with one finish coat of'Qualipur® 601 one -component, aliphatic polyurethane elastic coating. Cut out and repair up to 200 ft2 of the blisters found on the field and flashing membrane throughout the flat roofs to be coated with minimum 3' X 3' modified patches using GAF Ruberoid® Torch Granule membrane, heatwelded. 2. Power wash the entire surface of the roof and allow to thoroughly dry. 3. Seal all open base seams, gaps, voids and stress cracks found throughout the roofs using MasterSeal® NP I' one -component, elastomeric, polyurethane sealant 4. Apply one base coat of Qualipur® 601 one -component, aliphatic polyurethane elastic coating over the entire surface of the roof deck at'a coverage rate of 70 ft' per gallon (20 mils) and allow to cure overnight S. Any alteration or deviation from the agreed Scope of Work, with the exception of the removal and replacement of rotten wood, will be performed only upon written orders executed by an authorized party and will, if applicable, become an extra charge over and above the contract price. However, customer shall not give orders to Latite for work that is required to be performed and then refuse to make payment on the grounds that a change order was not executed at the time the work was performed or that customer's representative was not authorized to order the change. In such event, Latite shall be entitled to compensation representing the reasonable value of work performed. This agreement for performance of Work is contingent upon strikes, accidents or delays beyond our control. Owner/Customer shall carry fire, tornado and other necessary insurance. Latite workers are fully covered by Workman's Compensation Insurance. S. Partial payment shall be made as Work progresses. Net cash on completion unless otherwise indicated above. A draw schedule will be furnished on all construction exceeding'$5,000.00. Interest will be charged after maturity at the rate of 1.5% per month. Should customer fail to pay pursuant to the draw schedule, Latite reserves the right to suspend further performance of its contractual obligations until payment of all past due amounts are made. Should a dispute arise out of or in connection with this Agreement, then the prevailing party shall be entitled to all reasonable attorneys' fees and court costs in any administrative procedure, litigation or arbitration, at all levels including without limitation trial and appellate. In any matter involving payment for services and/or materials performed or furnished, the unpaid party shall be entitled to costs, expenses and attorneys fees incurred before suit. 6. Payment in full of all amounts hereunder shall be a condition precedent to the obligation of Latite or the manufacturer to issue any applicable bond or give any warranty hereunder to the owner. This term shall survive as a contractual obligation and be incorporated into the agreementof the parties. Latite will provide releases of liens based on statutory form prescribed by Section 713.20 Florida Statutes. 7. Latite. is not responsible for the quality of material or workmanship of other contractors or subcontractors whose work supersedes or follows that of Latite. Further, Latite is not responsible for preexisting conditions that tie into or affect the work, provided that the pre-existing condition is not open and obvious. Any warranty provided by or through Latite shall be void where the claimed failure of the Work is caused in whole or in part by preexisting conditions or the work of others. Preexisting conditions shall include but not be limited to buildings found to be structurally deficient, cracked slabs or other conditions or causes not within Latite's scope of work but that effecf the integrity of Latite's work. 8. Latite approaches everyjob in a professional manner and will make every effort to protect all finished surfaces as reasonably required and 'consistent with industry standards. Notwithstanding, any surface or item that Latite cannot protect, such as pool decks, driveways, walkways, lawns, outside furniture, vehicles, landscaping, sprinklers, etc., as well as interior walls, ceilings; floors, furnishings, and/or items affixed thereto, shall be the responsibility of the customer/contractor or owner to protect.as they deem fit. Latite assumes no responsibility for cracked or damaged driveways, sidewalks, curbing, interior surfaces, or other site work and shall not be liable for any interior damage sustained by owner after acceptance of Latite's work, or incidental or consequential damages to the building structure or its contents. Owner/Customer shall hold Latite harmless for interior damages sustained 'after acceptance of Latite'9 work, and shall look solely to its own insurance provider for coverage. Owner/Customer and anyone else on customer's behalf, waives all rights of subrogation with respect to claims made under any policy of insurance for damages described in this paragraph. Further, Latite disclaims any implied warranty, including the warranty of merchantability, and the warranty of fitness for a particular purpose, ancllimits any warranty to the duration and extent of the, express warranties provided for in this contract. 9: ANY CLAIMS FOR CONSTRUCTION DEFECTS ARE SUBJECT TO'THE NOTICE AND CURE PROVISIONS OF CHAPTER 558, FLORIDA STATUTES.' (FEC) has completed gravimetric tests on the eight roof samples delivered to our office. The tests were done in accordance with TAS- 126. This report transmits the results of our laboratory tests. - The subject building is located at 3000 North A-1-A in North Hutchinson Island, Florida. Eight roof samples, from four roof cores, were obtained from the roof of the subject building and delivered to our office in sealed plastic bags, with notations on the bags identifying the samples. We have used those same notations in our report. The'roof samples were removed from the bags and weighed. They were then placed in an oven and cooked at 230°F for 24 hours. The samples were then removed from the oven and weighed again. The loss in moisture was then computed. The laboratory moisture content test results are given in the attached sheet. (CIS) on the Atrium on the Ocean Condominium — Building Two, located at 3000 North Highway in Hutchinson Island, FL. PURPOSE OF THE SURVEY: Obtain visual and isotopic inspection data, to provide a non- destructive means to detect moisture by determining the amount of hydrogen within the layers of the roof using a nuclear (isotopic) Roof Moisture Gauge. The gauge neutrons can penetrate up to seven inches of roof and its coatings and the readings are of 7.5 seconds duration. METHOD: Hydrogen readings were taken at approximately five-foot intervals and the data recorded on the corresponding grid drawing. HYDROGEN READINGS: The purpose of the moisture test is to try to identify areas with wet roofing material. Following the field moisture tests, the customary procedure is for core samples to be taken at locations corresponding to readings that range from "low" to "high", in order to determine their moisture by weight content (gravimetric analysis). CODE CRITERIA: According to Florida Building Code (FBC) 2014, Chapter 15, Section 1521.4, not more than 25% of the total roof area or roof section of any existing building or structure shall be repaired, replaced or recovered in any 12 month period unless the entire existing roofing system or roof section is replaced or repaired to conform to requirements of this code. CORE SAMPLES: Core cuts were made to determine their moisture content (percentage by weight). Roof core samples, of a minimum of 2 inches in diameter, were taken down to the deck. The core samples were then transported to the laboratory in air -tight compartments to determine the type of roof components and percentage of moisture by weight. This graph separates moisture levels into four levels of dry to wet. The Florida Building Code sets allowable maximum limit of moisture in the roof system as 5% in the roof membrane and 8% in the insulation. (See chapter 15 section 1521.12) TEST SUMMARY: A total of 299 field nuclear readings were recorded. The Nuclear Gauge readings range from 12 to 59. Of the 299 readings, 100% of readings do not exceed the maximum allowable moisture by weight limit in the roof membrane (over 5%). (The Florida Building Code does not have a speck value for acceptable level of moisture for Lightweight Concrete). One Roof: Modified Membrane over Lightweight Concrete over Concrete Roof Deck. Roof meets current code for receiving roof repair or roof recover. However, water was found traveling between the roof membrane plys at Penthouse "D" — Column K @ Row 22 = 48. In addition all roof blisters should be repaired to current Florida Building Code. The Lightweight Concrete (LWC) was tested to measure the current moisture levels. The LWC moisture content ranged from 4.28% to 28.18%. The Florida Building Code does not have a specific value for acceptable level of moisture for Lightweight Concrete. However we refer to manufacturer's recommendation on allowable LWC moisture content. Of the 299 readings, 44 or 14.72% are above 18.79% moisture by weight content in the LWC. The test results presented here reflect the condition of the roof system at the time of the test. These results are time and sample dependent since roof conditions are continuously changing due to exposure to the elements and roof top traffic.
