It includes a schedule of Special Inspection Services applicable to this project as well as the name of the Special Inspector. The Special Inspector shall keep records of all inspections and shall furnish inspection reports to the Building Official, Owner, Engineer of Record and Contractor. All discrepancies shall be brought to the Contractor's attention, and if such discrepancies are not corrected, these discrepancies shall be brought to the attention of the Building Official and Structural Engineer of Record. The Special Inspector shall submit a final report to the Building Official documenting completion of all Special Inspections and corrections of any discrepancies prior to issuance of the Certificate of Occupancy. Special inspections do not relieve the Contractor of his or her responsibilities to carry out quality control inspections and testing. AM Ft. Pierce, Florida General Notes 1. The purpose of this special Inspection as required by the FBC, is to report and record the repair procedures highlighting those that are not in compliance with the Contract Documents. 2. The Special Inspector shall meet the requirements of the Board of Building Codes and Standards. The special inspector shall be qualified as a building inspector under Part 30I of Chapter 468, Florida Statutes, or certified as a special inspector under Chapter 471 or 481, Florida Statues 3. The Special Inspector must solely dedicate his time to the Project and shall not be allowed to provide other services or work, such as material or concrete testing on the same project. 4. The Contractor shall cooperate with and assist, however possible the Special Inspector in performing his inspections. The Contractor shall allow the Special Inspector access to the project at all times when work is in progress. The Contractor shall review this plan and coordinate the schedule of work such that a minimum of 24 hours notice is given to the Special Inspector before the work that requires inspection is performed. 5. The Special Inspector shall be responsible to the Owner and Building Department having jurisdiction for the project. 6. The Special Inspector shall perform site visits at regular intervals to ensure that the inspections are being performed by his on -site representative in accordance with the Threshold Plan. 7. The Special Inspector shall cooperate with the Contractor and shall immediately notify the Contractor of all discrepancies and deviations from the Contract Documents, and the Contractor shall immediately maker every effort to rectify said discrepancies or deviations. 8. Upon completion of the structural repairs, a signed and sealed statement by the Special Inspector must be submitted to the building department stating that the portion of the project under his inspection responsibilities and under the Threshold Plan was constructed in substantial conformance with the Contract Documents. 9. This document is in addition to the requirements of the standard building inspections as required by the Florida Building Code and is not meant to specify all inspections required by that Code. A —North Building balcony repairs at Units 201 & 301 SCANNED BY OBSERVATIONS:. St. LUCIQ County Unit 201 • The entire north and most of the east balcony edges require repair. • The reinforcing bars are corroded behind the PT anchors and will require about 8 PT cables at the north slab edge to be locked,off and spliced with new anchors. • The spalls at the doors extend under the door tracks, but these may be repaired without having to enter the unit. • The aluminum screen enclosure at Unit 101 below will require removal to repair the slab edge above. (Need an alternate price to replace with a new enclosure.) Unit 301 • There is a significant amount of ceiling spalls at the underside of the Unit 301 floor slab as well as surface spalls. Some of these areas will be full depth repairs if the repairs at the underside coincide with the upper surface spalls. • There are surface spalls along both sliding glass doors. • There is evidence of previous repairs where a blue -colored bonding agent is visible. • Consideration should be given to replace rather than repair the slab based on a cost comparison. RECOMMENDATIONS: Unit 201 • Continue the repairs as per the design plans. • Coordinate with the PT specialist to lock off the required cables. • Provide shoring during repairs as well as to re -shore Unit 301 above. • Provide a cost difference to remove and re -install the existing screen enclosure (with new screens, Florida panels and paint) vs. replacement with a new screen enclosure. Unit 301 • Provide a cost comparison to repair the slab and/or to replace the entire slab. Keep in mind that all of the PT cables will require replacement if the slab is repaired. A —North Building Balcony Repairs OBSERVATIONS: Units 201 &301 No work completed at this time Unit 608 Demolition of the deteriorated concrete at the north column was completed, bars cleaned and coated with corrosion inhibitor and column repaired. The concrete was removed at the spilled edge. It appears that is was repaired in the past and the material did not adequately bond with the surrounding original concrete. The exposed reinforcing bars at this area have minimal deterioration but should still be cleaned and coated with the specified corrosion inhibitor. Removal of the deteriorated concrete at the spall at the inside concrete column is in progress to fully expose the corroded reinforcing bars prior to cleaning and coating with corrosion inhibitor. RECOMMENDATIONS: Continue repairs as per the specs by mechanically cleaning all corroded reinforcing bars and coating with the specified corrosion inhibitor prior to applying concrete repair mortar. There is one splice that is not covered with a protective PVC which is to be placed. • Add 44 bar in the east west direction next to all bars that were cut to accommodate the cable lock off connection. Provide a minimum of 24" lap splice. • Remove miscellaneous abandoned aluminum handrail posts ends that were uncovered in the slab. • Cut ends of bars back to maintain a minimum of 2" concrete cover from slab edges. • Provide plastic chairs under the reinforcing mats to maintain the required concrete coverage RECOMMENDATIONS: If a ready mix concrete is going to be used for the slab pours instead of a Emaco 1061 EX bag mix repair mortar, a suitable bonding agent such as Aquafm is to be used as well as MasterSeal Traffic 1500 polyurethane water proofing system. (Need an alternate price to replace with a new enclosure.) Unit 301 • There is a significant amount of ceiling spalls at the underside of the Unit 301 floor slab as well as surface spalls. Some of these areas will be full depth repairs if the repairs at the underside coincide with the upper surface spalls. • There are surface spalls along both sliding glass doors. • There is evidence of previous repairs where a blue -colored bonding agent is visible. • Consideration should be given to replace rather than repair the slab based on a cost comparison. RECOMMENDATIONS: Unit 201 • Continue the repairs as per the design plans. • Coordinate with the PT specialist to lock off the required cables.. • Provide shoring during repairs as well as to re -shore Unit 301 above. • Provide a cost difference to remove and re -install the existing screen enclosure (with new screens, Florida panels and paint) vs, replacement with a new screen enclosure. Replace two damaged sections of screening in the screen enclosure of Unit 101. 2. Place sealant at the gap at the top rail and underside of slab. 3. Paint floor slab on grade. 4. Touch up paint the walls where required at Units 201 & 301. 5. Provide aluminum clip angle at the bottom rail to the comer post at Unit 201. 6.. Clean slab in front of the east SGD at Unit 201.
