General
- Dumpster and storage container at north side lot. Most equipment located at
south side lot.
- Shoring installed from ground to 5h floor, approximately every 4 feet with 2x4
below and 4x4 above.
- Lift being installed at walkway side of building.
- Excavations underway at 2d , P and 4h floors walkways.
- Scarifying / grinding of walkway coating underway at 5 th floor.
- Swing stages installed at east side of building. Painting at balcony stacks 10, 9, 8,
7 underway, south walls completed.

Inspection date(s): 9/21/18, 9/26/18, 9/27/18,10/l/18,10/3/18,10/5/18
We inspected the above referenced construction work. We observed the following:
- Concrete repairs work underway at 2 ad through 5h floors at walkways.
- Concrete placed at walkways at 2d through 5h floors at most locations.
- Partial depth repairs underway at 6"' floor north side elevator landing area.
- Some repair locations have significantly thinned reinforcing bars, and require lap splicing or dowels.
Some repair areas have limited rusted reinforcing bars.
- 506, 508, 509 loose and spalled concrete under door. Further excavations required.
- 4h floor masonry railing wall cap beam had significant cracking and spall damage. Damage extended
at entire wall cap beam. New continuous reinforcing bar is added, 2 # 5's top and bottom.
- 4h floor north landing near elevator needs ftirther excavation at slab above window (5h floor slab),
will protrude to exterior, likely accessible by swing stage in place, could be widespread at other floors.
- Column spall near 210 not yet started.
- 205 masonry railing wall beam excavation underway.
- Cracked/damaged electric conduit observed several locations on each floor. Electrician doing repairs.
- Saw -cuts are being used to square off repair areas. Okay but not required.
- Coating of bars discussed with contractor foreman. Do not over -coat the bars.
- Tool that was being used to clean bars was grinding the nubs of the rebar off at some locations. The
proper wire -wheel was discussed with contractor. Lap splice/doweIs added where required.
- Swing stage installed at 5/6 stack at east balcony side, painting u�derway at 5/6 stack and north wall.
Limited plastic protection is in place at 6 stack balcs floors 5,6,7,8. All floors painted at walkway near
south end near storage rooms

We insoected the above referenced construction work. We observed the following:
- Concrete repair work underway at 6t" floor at walkways.
- Full depth and partial depth repairs underway at 6th through gth floors north side elevator landing area.
Concrete placed at partial depth repairs at 2 d through 5h floors. North landing near elevator needs
further excavation at slab above window at 6th floor slab, 7th floor slab, and 8th floor slab. Repairs will
protrude to exterior, likely accessible by swing stage in place. Inadequate shoring observed at repair
areas at 6th floor, 7h floor, and 8d' floor slabs.
- At walkway, some repair locations have significantly thinned reinforcing bars, and require lap splicing
or dowels.. Some repair areas have limited rusted. reinforcing bars.

removed and location of new steel dowels verified. Concrete repairs satisfied with project's
specifications. New concrete placed.
- East Bldg — Audit check at previous concrete placement at 7fll floor repair locations. Concrete
removed and the locations of new steel dowels were not installed by contractor. Contractor
removed all new concrete placement at locations marked for new steel dowels. New dowels
