Ocean drive, Jensen Beach, FL 34957 Permit No. 1312-0243 Mathers Engineering Corporation visited the job site and made the following observations of the ongoing concrete restoration and repairs of exterior balconies: Site Observations from-4/7/15 thrn 4/29/15: Units 202 — 702- The only repairs made on these units were to caulk existing cracks along the balcony edges. • Units 203 — 703: The only repairs made on these units were to caulk existing cracks along the balcony edges. Unit 705: Site observations were made progressively during the repair phase. A complete full depth 12" edge repair was done for the entire width of the balcony. Steel reinforcement was inspected, cleaned and coated, and where necessary, new rebars were doweled and expoxied into existing concrete. Repair was subsequently waterproofed. Concrete restoration work on this unit is completed. Units 205 — 605: The only repairs made on these units were to caulk existing cracks along the balcony edges. Units 206 — 606: The only repairs made on these units were to caulk existing cracks along the balcony edges. • Units 207 — 607: The only repairs made on these units were to caulk existing cracks along the balcony edges. The repairs included a vertical column repair lying on the common wall between the two units. Steel reinforcement was inspected, cleaned and coated, and where necessary, new rebars were doweled and expoxied into existing concrete. Concrete restoration work on this column is completed. Units 208 — 608: The only repairs made on these units were to caulk existing cracks along the balcony edges. • Unit 708: Site observations were made progressively during the repair phase. A complete full depth 13" edge repair was done for the entire width of the balcony. Steel reinforcement was inspected, cleaned and coated, and where necessary, new rebars were doweled and expoxied into existing concrete. Repair was subsequently waterproofed. Concrete restoration work on this unit is completed. Unit 410: Site observations were made progressively during the repair phase. The only repairs were a small edge repair at a guard rail post. Concrete restoration on this unit is completed. Unit 509: Site observations were made progressively during the repair phase. The repairs included a partial depth repair near the sliding glass doors. Repair was subsequently waterproofed. Concrete restoration work on this unit is completed. • Unit 607: Site observations are ongoing. The balcony edge is undergoing a repair. Concrete restoration work on this unit is not completed. Unit 507: Site observations indicated areas of balcony deck near living room sliding glass doors that were cracking. Contractor was instructed to chip out area for inspection. Partial depth repairs were made. Steel reinforcement was inspected, cleaned and coated, and where necessary, new rebars were doweled and expoxied into the existing slab. Concrete repair was made using 5,000 psi concrete bag mix. Repair area was subsequently waterproofed. Existing guardrails were removed and new guard rails will be installed by a separate railing company under separate permit. Contractor was instructed to chip out area for inspection. Partial depth repairs were made. Steel reinforcement was inspected, cleaned and coated, and where necessary, new rebars were doweled and expoxied into the existing slab. Concrete repair was made using 5,000 psi concrete bag mix. Repair area was subsequently waterproofed. Existing guardrails were removed and new guard rails will be installed'by a separate railing company under separate permit. Unit 609: Site observations indicated areas of balcony deck near the bedroom sliding glass doors were cracking. In addition, cracking was observed along the balcony edge. Contractor was instructed to chip out near the sliding glass doors and to chip off the balcony edge for inspection. A complete full depth 6-inch repair was made, and apartial depth 6-inch repair behind the full depth repair was made for the full length of the edge. Partial depth repairs were made by the bedroom sliding glass doors. Steel reinforcement was inspected, cleaned and coated, and where necessary, new rebars were doweled and expoxied into the existing slab. Concrete repair was made using 5,000 psi concrete bag mix. Repair area was subsequently waterproofed. Existing guardrails were removed and new guard rails will be installed by a separate railing company under separate permit. • Units 201 thru 701: (Stack at the North end of the building) Site observations indicated cracking along all balcony edges. Contractor was instructed to start chipping away at the edge until good rebar was found. The same instructions were given for Unit 401. After extensive test chipping, it was determined that Unit 201 and 401 needed "complete removal. Based on these observations and with Association approval, it may determined that Units 201 thru 701 will have full deck replacement, unless it is determined during demolition that the existing condition of the steel reinforcement will allow for deck repairs. Existing building's components and elements outside the renovation area of this permit do not have their current wind load resistant capacity or gravity load capacity compromised by the completed concrete restoration. Mathers Engineering Corporation performed onsite inspections and reports to the building department as the job progressed. Repairs required minor shoring. Work such as concrete repair and addition of steel reinforcement was inspected. The enclosed Record Drawings (Sheets 1, 2 and 3) show the areas of restoration and repair, and are made part of this final close-out document approval. At the time the work was completed, Unit 201 had a full depth repair and the work has been completed. The unit owner has failed to install new shutters. The Association has mandated that the owner of Unit 201 board up the balcony deck during the entire hurricane season until they install new shutters. Materials used an work performed to date has been substantially completed in conformance with the specifications outlined in the IRCI Guideline No. 03730, as well as the applicable 2010 Florida Building Codes and updates, other industry guidelines, or standard engineering practices.
