Midway Road Dear Mr. Harrison, CATHYTOWNSEND VICE -CHAIR The Water Quality Division has reviewed the revised drainage calculations dated January 30, DISTRICT 5 2019, the revised plans dated February 20, 2019 and has received the Storrawater fee for the above referenced project. The Storrawater permit application for the above referenced project CHRis DZADOVSKY is hereby deemed complete and the construction of site improvements approved DISTRICT I conditioned on the following: 1. Applicant shall schedule a pre -construction meeting with the Contractor, County Water SEAN MITCHELL Quality Division, and the Engineer of Record (EOR). No work shall commence prior to the DISTRICT 2 pre -construction meeting. 2. A set of plans stamped 'Stormwater Approved' as accepted by the County, together FRANNIE HUTCHINSON with all other applicable permits shall be on site at all times during construction. DISTRICT 4 3. All other applicable agency and county permits must be obtained prior to commencement of work. The more stringent condition of common or conflicting conditions shall be met. Discrepancies within the plans shall be appropriately addressed HOWARD N. This includes TDD material changes such as pavement type, type of drainage pipe, etc. (772) 462-1428 7. Any and all structures shown or not shown on the plans (such as walls, gates, signs, columns, etc.) may require building permit(s). The applicant is solely responsible for FAX obtaining other permit(s). This permit (Stormwater permit 18-09) does not authorize the (772) 462-2363 construction of above ground structures, 8. Any and all density test results, proctor tests and other related information shall be included in the certification package. Any substantial deviations from the approved plans shall be noted in the certification document. Final certification must demonstrate that the constructed facilities function and perform as per the approved design or better. 9. A published benchmark utilized for site elevations shall be referenced for verification by other parties, including but not limited to the County. 10. Elevations at and along all stormwater management facilities must be shown on the as -built plans. The plans must depict, at a minimum, the location and finished grades of all building corriers, critical pavement spot elevations, and changes in grade or slope. The location, type, size, material and inverts of all drainage structures and pipes, the top of bank, toe of slope and detail of control structure(s) for any and all Stonriwater treatment facilities shall also be depicted. Sufficient elevations for all accessible routes shall be provided to demonstrate compliance with all applicable regulations. 11. Any and all disturbed areas within the Right of Way must be graded to provide positive drainage and stabilized/sodded immediately upon completion of final grading. 12. Substantial completion shall require that onsite areas shall be stabilized/sodded to provide a uniform, evenly distributed, without large bare areas of perennial vegetative cover with a density of at least 70% for all unpaved areas and areas not covered by permanent structures. This shall be achieved prior to removal.of sedimentation controls. 13. Please be advised, operators of construction activities that propose to clear, excavate or fill 1.0 acre of land or more over the life of the project are required to obtain coverage Linder the Florida Department of Environmental Protection's National Pollutant Discharge Elimination System "Generic Permit for Stormwater Discharge from Large and Small Construction Activities". The Notice of Intent (NOI) to use the generic permit requires a Stormwater Pollution Prevention Plan (SWPPP) that must be implemented to show compliance. Elements of a SWPPP include Best Managoement Practices that will be utilized to minimize soil erosion and sedimentation from degrading the water quality of downstream water bodies for any size project including less than 1.0 acre of disturbance.
