Our field observations were based on the permit documentation and plan modifications dated 03/22/2019. Permit documentation and plan modifications were prepared by the engineer of record, Brian Maskol, P.E. of Summit Design and Forensics. Site Observations from 03/19/2019 to 04/08/2019: • On 03/19/2019, observed demolition to remove old window glass. (Pass) • On 03/26/2019, observed initial installation CMU pre -pour inspection. (Pass) • On 04/08/2019, final inspection. William J. Mathers, P.E., S.I. #155, will certify all structural inspections. Under Florida Statute 553.79(5)(d), "the licensed registered engineer service as the special inspector shall be permitted to send his duly authorized representative to the job site to perform the necessary inspections provided al written reports are prepared by and bear the seal of the special inspector and are submitted to the enforcement agency."
