Response: Understood, GC will provide all of the subcontractor information to the county.
3. Comment: NEED VEG PERMIT
Response: Understood, all required permits will be applied for.
4. Comment: GC LICENSE NUMBER ON THE APPLICATION IS FROM LEROY
HOLLINGSWORTH AND NOT FROM KEITH SMITH. EITHER KEITH SMITH WILL
NEED TO REGISTER OR LEROY HOLLINGSWORTH WILL NEED TO UPDATE HIS
LICENSE. FIRST PAGE APPLICATION WILL NEED TO REFLECT THE CORRECT NAME
OF THE GC.
Response: Understood, a letter of authorization will be provided by Leroy Hallingsworth to
give Keith Smith permission to facilitate all GC requirements for this project.
5. Comment: ACCORDING TO PROPERTY APPRAISER RECORDS AND THE SUBMITTED
WARRANTY DEED, VRE FT PIERCE MIDWAY IS NOT THE PROPERTY OWNER. DID

THE PERMIT WILL NOT BE ISSUED UNTIL SITE PLAN APPROVAL HAS BEEN
