This NOA is being issued under the applicable rules and regulations go�,Ali Uypol truction materials. -The
documentation submitted has been reviewed and accepted by Miami-Dadc County M=t Control Section to
be used in Miami Dade County and other areas where "owed by the Authority Having Jurisdiction (AIIJ).
This NOA shall not be valid after the expiration date stated below. 1he Miami -Dade County Product Control Section
(In. Miami Dade County) and/or the AHJ (in areas other than Miami Dade County) reserve the right to have this
product or material tested for quality assurance purposes. If this product or material fails to perform in the accepted
manner, the manufacturer will incur the expense of such testing and the AHJ may immediately revoke, modify, or
suspend the use of such product or material within their jurisdiction. RER reserves the right to revoke this
acceptance, if it is determined by Miami -Dade County Product Control Section that this product or material fails to
meet the requirements of the, applicable building code.
This product is approved as described herein, and has been designed to comply with the Florida Building Code,
including the High Velocity Huaicane Zone. File Copy

Door", sheets I through 3 of 8,.prepai�ed by AL-Farooq Corporation, dat.ed 09/10/04 and last revised on DEC 07,
2017, signed and scaled by Javad Ahmad, PE., bearing the Miami -Dade County Product Control Revision stamp
with the Notice of Acceptance number and expiration date by the Miami -Dade County Product Control Section.
AMISSII h1PACT RATING: Small Missile Impact Resistant
Limitations:
I . See Design Pressure (DP) ratings with glass types & Reinforcing in sheet 2. See DP ratings of anchor types in
sheet 3. Lower Desigrt Pressure Ratings shall apply to entire system. Positive (Exterior) DP shall be limited to
sill height per sheet 2
2. The total tested finnie area must not exceed 180 sq. ft. per F.BC requirements.
3. All SGD doors containing operable pawl ( X) require channel reinfbrcement (item 43 1) at Bottom Rails.
LABELING: Each unit shall bear a permanent label with the manufacturer's name or logo, city, state, madel/series,
and following statement: "Miami -Dade County Product Control Approved", unless otherwise noted herein -
RENEWAL of this NOA shall be considered after a renewal application has been filed and there has been no
change in the applicable building code negatively affecting the performance of this product
TERMINATION of this NOA will occur afLer the expiration date or ifthere has been a revision or change in the
materials, use, and/or manufacture of the product or process. Misuse of this NOA as an endorsement of any produc4
for sales, advertising or any other purposes shall automatically terminate this NOA. Failure to comply with any
section of this NTOA shall be cause for termination and removal of NOA.
ADVERTISEMENT: The NOA number preceded by the words Miami -Dade County, Florida, and followed by �ffie
expiration date may be displayed in adverdsing literature. If any portion of the NOA is displayed, then it shall be
done in its entirety.
INSPECHON: A copy of this entire NOA shall be provided to the, user by the manufacturer or its distributors and
shall be available for inspection at the job site at the request of the Building Official. -
This NOA revises NOA #14-0807.18 and consists of this page I and evidence pages E-I and E-2, as well as
approval document mentioned above.

Any unforeseen circumstances can be addressed through the Project Manager and will be
remedied through a change order.
2. Island Kitchen And Bath is not responsible for removal and reinstallation osf Shutters
that could or will intefere with the installation ofsliders or windows. Homeowner will be
responsible for having shutters removed and reinstalled
Usland Kitchen And Bath is not responsible for the removal and reinstallation of window
treatments or blinds.
4.1sland Kitchen and Bath is not responsible for Alarm system removal and installation.
5. For drywall and Stucco repairs'outside of, normal scope see note #1
6. Island Kitchen and Bath is not responsible for delays from Engineering, Permiting, extended
wait times on orders placed due to Manufacture Back log

TERMS AND CONDITIONS
— This estimate is valid for 30 calendar days starting on the date stated at the top of the estimate.
— The prices represented ane based on project and product average costs. Prices may vary once project and Product measurements, details, and
selections are made by client
— ISLAND IGTCHEN AND BATH will execute all work according to the Florida Building Code and is licensed in the state of Florida and is in good
standing. Time spent by the owners on the project is not a billable item. All work will be accordance with the scope of work detalledwithin the
contract
— Due to unforeseeable conditions inside walls, behind cabinets, etc.., additional cost may occur during construction for issues such as mold, and
e)dsting plumbing/ electrical not being safe or code compliant These additional costs am NOT included in my contract pricing and will be dealt
with as needed. This contract does NOT account for product defects or product backorder. These issues may affect the project timeline and will
be dealt with as needed.
— Any changes requested by the client, after the original contract is signed, will result in a change order. Change orders will affect the project
timeline and are an additional cost to the original contract pricing. A change order will result in a separate fee of $100 per change order, in
addition to the total cost of the Change Order. Change Orden must be signed, and paid immediately and separately from the original contract
pricing,
— All selections must be made at the time of the contract, unless otherwise stated, such as,'To Be Decided (TBD) or Supplied by Owner (SBO). If
there is not a selection stated, TBD, or SBO thenwe cannot promise the price written within the contract
— It is the clienes responsibility to provide all additional information that may be needed for the project Such as, appliance specifications, all
SBO material, etc. If any incorrect information is provided by the client Island Fitchen and Bath is NOT responsible for any additional cost or
time that may occur from this error.
