Hi John, SCANNED
St. Lucie Count
I forgot to mention, You will only have to submit page 2 and page 3 of the application for each permit number and not
the complete application. Page 3 is required to have original signatures. The rest of the info on the application stays the
same so should not have to be filled out again.
Thanks,

Subject: RE: Permit 1402-0247 north wall main entrance.
Good morning John,
I spoke with contractor licensing supervisor and the plans examiner. They informed me that you are allowed to do
some concrete work on signs and have a masonry contractor as a sub, however, as soon as the masonry and structural
part is more than the value of the sign the GC should be the main contractor. We will need to change all the applications
to Holeman Inc as the GC with you and the electrical contractor as sub -contractors. What, you will need to do is submit 6
applications signed by holeman Inc. and 6 sub agreements signed by yourself. We will use the same permit numbers, the
plan review fee doesn't change and the permit will keep on moving through the review process while waiting for the
above documentation. I am going to work on the other 4 permits today and if I have comments l will send you those as
soon as possible. Please bring the requested information to me so I can make the changes in the permits. You will not
have to wait in line at the permitting department.
We apologize for the inconvenience. If you have any questions, please let me know. I will be in the office today until
12:30pm.
Thanks,

HI John,
Yes, you only need 4 filled lands affidavits.
NOC's normally are submitted at time of pickup of the permit or in, some cases, as first inspection,
however, I still need to list them as missing document.
Yes, you need 6 more surveys. I haven't finished the review for the other 4 so, please wait until you have
all reviews before you resubmit. I did notice that some changes need to be made.
I was informed by the plans examiner that you were not able to do all the work. I will double check with
contractor licensing what you are allowed to do under you license and I will let you know.
I will get back to you on Monday with more info and the rest of the reviews.

Hi Lydia,
Am I effectively going to have 4 permits that require filled lands affidavits (the existing boundary
walls are only being modified by stucco finish, paint, cast stone cap and sign letters)? Or will you be
wanting 6?
I am aware that the permits require all require "NOC" but as I understood it had to be before first
f ih pection.
And I understand you'll need 6 more sealed surveys...
I assumed since I can pour concrete and steel with my license that I could hire Holeman to do that scope
of work for me. I have done it else where ... please explain.
Thanks John Burrus
