Please complete all information in the space provided. All information must be printed (use black or blue ink
only) or typed. This roof permit application is to be used only for those activities that are not otherwise included
under a primary building permit. This application may not be used for any activity that includes structural
alteration. Building activities involving structural alteration, in addition to the roof work, must be permitted
through the regular building permit review process. The information to be provided with this application
includes:

Indicate the street address or general location of the
property where the building activity is taking place.
Indicate the tax identification number of the property where
the building activity is taking place.
Completely describe the building activity to take place.
Please indicate if tear -off or shingle over shingle etc..
Indicate total roof area to be affected by this permit. Also
indicate the pitch of the roof, expressed as a standard ratio
of run rise.
Indicate the type of roof being repaired or resurfaced.
Indicate the name and address of the owner of the property
on which building activity is taking place.
Indicate the State of Florida registration number (if
applicable), St. Lucie County contractor license number and
the name of the business. doing the work.
Indicate the total value of tfie work to take place. Total cost
of construction includes all material and labor costs
associated with the building/construction activity.
Construction value is used to determine the permit fee. St.
Lucie County reserves the right to question and/or modify
the indicated value of construction if it is demonstrated that

of construction.
This application can be submitted to St. Lucie County Building and Zoning, 2300 Virginia Avenue, Fort Pierce,
FL 34982. All permit applications must be filled out completely before submission. No applications will be
accepted for processing after 4:30 P.M. For assistance in completing this application, please call (772)
462-1553, during regular office hours (8:00 AM - 5:00 PM), Monday through Friday.
Upon issuance of this permit, required inspections can be scheduled by calling (772) 462-1261.

OWNER/BUILDER AFFIDAVIT DISCLOSURE STATEMENT
F.S. 489.103 (7) EXEMPTIONS
State law requires construction to be done by licensed contractors. You have applied for a permit under an
exemption to that law. The exemption allows you, as the owner of your property, to act as your own contractor even
though you do not have a license. You must provide direct, on -site supervision of the construction yourself. You
may build or improve farm outbuildings, a one -family or two-family residence or a commercial building at a cost of
under $75,000.00. The building or residence must be for your own occupancy. It may not be built or substantially
improved for sale or lease. If you sell or lease a building you have built or substantially improved your self within
one year after the construction is complete, the law will presume that you built or substantially improved it for gale
or lease, which is.a violation of this exemption. You may not hire an unlicensed person to act as your contractor or
to supervise people working on your building; it is your responsibility to make sure that people employed by you
have licenses required by state law and by county or municipal licensing ordinances. You may not delegate the
responsibility for supervising work to a licensed contractor who is not licensed to perform the work being done.
Any person working on your building who is not licensed must work under your direct supervision and must be
employed by you, which means that you must deduct F.I.C.A. and withholding tax and provide workers'
compensation for that employee, all as prescribed by law. Your construction must comply with all applicable laws,
ordinances, building codes, and zoning regulations. Initial :? !/�
I understand that the building official and inspectors are not there to design or give advice on o cet

I understand that as an owner -builder that any contract disputes with sub -contractors and I must be handled
in a civil court with the advice of an attorney. This department will not mitigate any contract disputes.

SCOPE:
']'his NOA is being issued under the applicable rules and regulations governing the use of construction materials. The
documentation submitted has been reviewed and accepted by Miami -Dade County RER - Product Control Section to be
used in Miami Dade County and other areas where allowed by the Authority Having Jurisdiction (AHJ).
This NOA shall not be valid after the expiration date stated below. The Miami -Dade County Product Control
Section (In Miami Dade County) and/or the A.HJ (in areas other than Miami Dade County) reserve the right to
have this product or material tested for quality assurance purposes. If this product or material fails to perform in
the accepted manner, the manufacturer will incur the expense of such testing and the A.HJ may immediately
revoke, modify, or suspend the use of such product or material within their jurisdiction. RER reserves the right to
revoke this acceptance, if it is determined by Miami -Dade County.Product Control Section that this product or
material fails to meet the requirements of the applicable building code.
This product is approved as described herein, and has been designed to comply with the Florida Building Code
including the High Velocity Hurricane Zone of the Florida Building Code.

LABELING: Each unit shall bear a permanent label with the manufacturer's name or logo, city, state and following
statement: "Miami -Dade County Product Control Approved", unless otherwise noted herein.
RENEWAL of this NOA shall be considered after a renewal application has been filed and there has been no change
in the applicable building code negatively affecting the performance of this product.
TERMINATION of this NOA will occur after the expiration date or if there has been a revision or change in the
materials, use, and/or manufacture of the product or process. Misuse of this NOA as an endorsement of any product,
for sales, advertising or any other purposes shall automatically terminate this NOA. Failure to comply with any section
of this NOA shall be cause for termination and removal of NOA.
ADVERTISEMENT: The NOA number preceded by the words Miami -Dade County, Florida, and followed by the
expiration date may be displayed in advertising literature. If any portion of the NOA is displayed, then it shall be done
in its entirety.
INSPECTION: A copy of this entire NOA shall be provided to the user by the manufacturer or its distributors and
shall be available for inspection at the job site at the request of the Building Official.

8. All nails in the deck shall be carefully checked for protruding heads. Re -fasten any loose decking panels. Sweep
the deck thoroughly to remove any dust and debris prior to application. Install when temperatures are 45 degrees
and rising.
9. When applying the membrane in the valley, start at the low point and work to the high point, rolling the membrane
from the center outward in both directions.
10. Roll or broom the entire membrane surface so as to have 100% contact with the surface, giving special attention to
overlap areas.
11. Flash vent pipes, stacks, chimneys and penetrations in compliance with Roof Assembly current Product Control
Notice of Acceptance and applicable Building Code.
12. Wherever the adhesive back of UnderRooff 2 Waterproof Protection, or UnderRooff HT High Temperature
Leak Barrier comes in contact with polyester surfacing, such as end laps, penetrations, and flashing material, it
must be set in a 1/8 inch troweling of LeakBusterm Matrix® 201 Premium SBS Flashing Cement made in
accordance with ASTM D4586-Type I or equal flashing cement.
13. All products listed herein shall have a quality assurance audit in accordance with the Florida Building Code and
Rule 9N-3 of the Florida Administrative Code
14. All membranes or packaging shall bear the imprint or identifiable marking of the manufacturer's name or logo and
the following statement: "Miami -Dade County Product Control Approved" or the Miami -Dade County Product

SCOPE:
This NOA is being issued under the applicable rules and regulations governing the use of construction
materials. The documentation submitted has been reviewed and accepted by Miami -Dade County PERA
- Product Control Section to be used in Miami Dade County and other areas where allowed by the
Authority Having Jurisdiction (AHJ).
This NOA shall not be valid after the expiration date stated below. The Miami -Dade County
Product Control Section (In Miami Dade County) and/or the AHJ (in areas other than Miami Dade
County) reserve the right to have this product or material tested for quality assurance purposes. If
this product or material fails to perform in the accepted manner, the manufacturer will incur the
expense of such testing and the AHJ may immediately revoke, modify, or suspend the use of such
product or material within their jurisdiction. PERA reserves the right to revoke this acceptance, if it
is determined by Miami -Dade County Product Control Section that this product or material fails to
meet the requirements of the applicable building code.
This product is approved as described herein, and has been designed to comply with the Florida Building
Code including the High Velocity Hurricane Zone of the Florida Building Code.
DESCRIPTION: GAF Deck -Armor At Underlayment
LABELING: Each unit shall bear a permanent .label with the manufacturer's name or logo, city, state
and following statement: "Miami -.Dade County Product Control Approved", unless otherwise noted
herein.
RENEWAL of this NOA shall be considered after a renewal application has been filed and there has
been no change in the applicable building code .negatively affecting the performance of this product.
TERMINATION of this NOA will occur after the expiration date or if there has been a revision or
change in the materials, use, and/or manufacture of the product or process. Misuse of this NOA as an
endorsement of any product, for sales, advertising or any other purposes shall automatically terminate
this NOA. Failure to comply with any section of this NOA shall be cause for termination and removal of
NOA.
ADVERTISEMENT: The NOA number preceded by the words Miami -.Dade County, Florida, and
followed by the expiration date may be displayed in advertising literature. If any portion of the NOA is
displayed, then it shall be done in its entirety.
INSPECTION: A copy of this entire NOA shall be provided to the user by the manufacturer or its
distributors and shall be available for inspection at the job site at the request of the Building Official.

2. Deck-ArmorT"' shall not be used as roof tile underlayment or anchor sheet within a roof tile
underlayment system.
3. This acceptance is for prepared roofing applications. Minimum deck requirements shall be in

installed in strict compliance with applicable Building Codes and Manufacturers' specifications and
