This application for replacement of Windows, Skylights, and Doors is to be used only for those activities that are not otherwise included
under a primary building permit. This application for Permit may not be used for any activity that includes any type or kind of structural
alteration. Building activities involving structural alteration, in addition to the replacement of Windows, Skylights, Doors & Overhead
Garage Doors, must be permitted through the regular building permit review process. The information to be provided with this application
includes:

All applications for Permits are to be submitted to the St Lucie County Building and Zoning Department; St Lucie County Administration
Building, 2300 Virginia Avenue, Fort Pierce, FL 34982. All applications for Permits must be completed and filed with the Department no
later than 4:30 P.k each business day. No applications will be accepted for processing after 4:30 P.M. For assistance in completing
this application, please contact the St Lucie County Building and Zoning Department, at (772) 462-1553, during regular off ce hours (8:00
AM - 5:00 PM), Monday through Friday.
Following the issuance of this Windows, Skylights, Doors & Overhead doors replacement Permit, the scheduling of all required inspections
may be made by calling (772) 462-1261.

OWNER/BUILDER AFFIDAVIT DISCLOSURE STATEMENT
F.S. 489.103 (7) EXEMPTIONS
State law requires construction to be done by licensed contractors. You have applied for a permit under an
exemption to that law. The exemption allows you, as the owner of your property, to act as your own contractor even
though you do not have a license. You must provide direct, on -site supervision of the construction yourself. You
may build or improve farm outbuildings, a one -family or two-family residence or a commercial building at a cost of
under !$75,000.00. The building or residence must be for your own occupancy. It may not be built or substantially
improved for sale or lease. If you sell or lease a building you have built or substantially improved your self within
one year after the construction is complete, the law will presume that you built or substantially improved it for sale
or lease, which is a violation of this exemption. You may not hire an unlicensed person to act as your contractor or
to supervise people working on your building; it is your responsibility to make sure that people employed by you
have licenses required by state law and by county or municipal licensing ordinances. You may not delegate the
responsibility for supervising work to a licensed contractor who is not licensed to perform the work being done.
Any person working on your building who is not licensed must work under your direct supervision and must be
employed by you, which means that you must deduct F.I.C.A. and withholding tax and provide workers'
compensation for that employee, all as prescribed by law. Your construction must comply with all applicab}g laws,
ordinances, building codes, and zoning regulations. Initial ;{ C

SCOPE•
This NOA is being issued under the applicable rules and regulations governing the use of construction
materials. The documentation submitted has been reviewed and accepted by Miami -Dade County PERA -
Product Control Section to be used in Miami Dade County and other areas where allowed by the Authority
Having Jurisdiction (AHJ).
This NOA shall not be valid after the expiration date stated below. The Miami -Dade County Product Control
Section �(In Miami Dade County) and/or the AHJ (in areas other than Miami Dade County) reserve the right
to have', this product or material tested for quality assurance purposes. If this product or material fails to
perforin'j in the accepted manner, the manufacturer will incur the expense of such testing and the AHJ may
immediately revoke, modify, or suspend the use of such product or material within their jurisdiction. PERA
reserves, the right to revoke this acceptance, if it is determined by Miami -Dade County Product Control
Section that this product or material fails to meet the requirements of the applicable building code.
This product is approved as described herein, and has been designed to comply with the Florida. Building
Code, including the High Velocity Hurricane Zone.

APPROVAL DOCUMENT: Drawing No. L8700-0901, titled "HS-8700 Horizontal Rolling Flange
Impact Window", sheets 1 through 10 of 10, dated 05/30/09 with revision A dated 01/25/12, prepared by
manufacturer, signed and sealed by Thomas J. Sotos, P. E., bearing the Miami —Dade County Product Control
Revision' stamp with the Notice of Acceptance number and expiration date by the Miami —Dade County
Product Control Section.

LABELING: Each unit shall bear a permanent label with the manufacturer's name or logo, city, state,
model/series, and following statement: "Miami -Dade County Product Control Approved", unless otherwise
noted herein.
RENEWAL of this NOA shall be considered after a renewal application has been filed and there has been
no change in the applicable building code negatively affecting the performance of this product.
TERMINATION of this NOA will occur after the expiration date or if there has been a revision or change
in the materials, use, and/or manufacture of the product or process. Misuse of this NOA as an endorsement
of any product, for sales, advertising or any other purposes shall automatically terminate this NOA. Failure
to comply with any section of this NOA shall be cause for tennination and removal of NOA.
ADVERTISEMENT: The NOA number preceded by the words Miami -Dade County, Florida, and
followed by the expiration date may be displayed in advertising literature. If any portion of the NOA is
displayed; then it shall be done in its entirety.
INSPECTION: A copy of this entire NOA shall be provided to the user by the manufacturer or its
distributors and shall be available for inspection at the job site at the request of the Building Official.
