This application is hereby made to obtain a permit to do the work and installations as indicated, and to obtain a certificate of capacity,
if applicable, for the permitted work. I certify that no work or installation has commenced prior to the issuance of a permit and that all
work will be performed to meet the standards of all laws regulating construction in this jurisdiction. I understand that separate permits
may be required for ELECTRICAL, PLUMBING, SIGNS, WELLS, POOLS, FURNACES, BOILERS, HEATERS, TANKS,
AND AIR CONDITIONERS, FENCES, ETC., not otherwise included with this building permit application.
St. Lucie County makes no representation that its granting of a permit will authorize the permit holder to build the subject structure
which is in conflict with any applicable Homeowner Association rules, bylaws or any covenants that may restrict or prohibit such

F.S. 489.103 (7) EXEMPTIONS
State law requires construction to be done by licensed contractors. You have applied for a permit under an
exemption to that law. The exemption allows you, as the owner of your property, to act as your own contractor even
though you do not have a license. You must provide direct, on -site supervision of the construction yourself. You
may build or improve farm outbuildings, a one -family or two-family residence for your use and occupancy. You
may also build or improve a commercial building at a cost not exceeding $75,000.00 as long as it is for your own
use or occupancy. You may not build or improve said structures for the purposes of selling or leasing that building.
If you sell or lease a building you have built or improved within one year after construction is complete, then a
presumption is created that it was built or improved for sale or lease, which is a violation of this exemption. You
may not hire an unlicensed person to act as your contractor or to supervise people working on your building; it is
your responsibility to make sure that people employed by you have licenses required by state law and by county or
municipal licensing ordinances. You may not delegate the responsibility for supervising work to a licensed
contractor who is not licensed to perform the work being done. Your construction must comply with all_�pplicab�e

SCOPE:
This NOA is being issued under the applicable riles and regulations governing the use of construction materials. The
documentation submitted has been reviewed and accepted by Miami -Dade County RER-Product Control Section to be
used in Miami Dade County and other areas where allowed by the Authority Having Jurisdiction (AHJ).
This NOA shall- not be valid after the expiration date stated below. The Miami -Dade County Product Control Section
(In Miami Dade County) and/or the AHJ (in areas other than Miami Dade County) reserve the right to have this
product or material tested for quality assurance purposes. If this product or material fails to perform in the accepted
manner, the manufacturer will incur the expense of such testing and the AHJ may immediately revoke, modify, or
suspend the use of such product or material withinn their jurisdiction. RER reserves the right to revoke this acceptance,
if it is determined by Miami -Dade County Product Control Section that this product or material fails to meet the
requirements of the applicable building code.
This product is approved as described herein, and has been designed to comply with the Florida Building Code,
including the High Velocity Hurricane Zone.
DESCRIPTION: "Hardie", "Cem" and "Prevail" Planks and Panels Fiber Cement Siding and Soffit
APPROVAL DOCUMENT: Drawing No. PNL, PLK & SOFF, titled "HardiePanel, CemPanel, Prevail
Panel; HatdiePlank, CemPiank Prevail Lap Siding; HardieSoffit, CemSoffit Panel; Installation Details
Wood/Metal Stud Construction", sheets 1 through 12 of 12, dated 04/24/2013, prepared by the
manufacturer, signed and sealed by Ronald 1.Ogawa, P.E., bearing the Miami -Dade County Product
Control revision stamp with the Notice of Acceptance number and expiration date by the Miami -Dade
County Product Control Section.
MISSILE IMPACT RATING: Large and Small Missile Impact Resistant
LABELING: A permanent label with the manufacturer's naive or logo, Plant City, Florida, and the
following statements: "ASTM C 1186 Type A compliant" and "Miami -Dade County Product Control
Approved" is to be located on each siding plank or panel and per FBC 1715.9.2 and 1715.9.3 on soffit
panels.
RENEWAL of this NOA shall be considered after a renewal application has been filed and there has been no change
in the applicable building code negatively affecting the perforniance of this product.
TERMINATION of this NOA will occur after the expiration date or if there has been a revision or change in the
materials, use, and/or manufacture of the product or process. Misuse of this NOA as an endorsement of any product,
for sales, advertising or any other purposes shall automatically tenninate this NOA. Failure to comply with any section
of this NOA shall be cause for termination and removal of NOA.
ADVERTISEMENT: The NOA number preceded by the words Miami -Dade County, Florida, and followed by the
expiration date may be displayed in advertising literature. If any portion of the NOA is displayed, then it shall be done
in its entirety.
INSPECTION: A copy of this entire NOA shall be provided to the user by the manufacturer or its distributors and
shall be available for 'inspection at the job site at the request of the Building Official.

1. This product has been evaluated and is in compliance with the 2010 Florida Building Code
(FBC) structural requirements including the "High Velocity Hurricane Zone" (HVHZ).
2. Product anchors shall be as listed and spaced as shown on details. Anchor embedment to
base material shall be beyond wall dressing or stucco.
3. When used in the "HVHZ' this product is required to be protected with an impact resistant
covering that complies with Section 1626 of the FBC.

4. When used in areas outside of the "HVHZ" requiring wind bome debris protection this product
is required to be protected with an impact resistant covering that complies with Section
1609.1.2 of the FBC.
