INFORMAL MEETING

Jim Davis, EMI (Employers Mutual Inc. formerly McCreary Corp)
The presentation began with a brief overview of the medical trends over the past few years.
The medical trend was explained as being a combination of both rising medical costs and
utilization (combining medical with Rx.) based on a per employee/per month average.
Percent of increase/decrease annually
Between 2003-2005 -medical trend increased by 7.1%
2005-2006 Increased up to 15.7% (transition was a factor)
2006-2007 Was basically flat - due to the fact that they sold their TPA business to Web TPA
and during that one month of transition (September 2005) ,the claims were not paid in this
year, making the trend higher the following year. Claims that should have been paid in 2004-
2005 were processed in 2005-2006
2007-2008 increased to 14.2%
2008-2009 slightly negative
County health overall running at 7.4% over 5 years compared to state average which is 12%.
National (10.5% this year) and State of FL is 12% forecast for 2010. Trend fluctuates based on
geography.
Clinic Presentation /Executive Summary
He state that an analysis was made of two completely different clinic comparison model
proposals. (see attached)
Two clinic models:
Care Here Crown Consulting
Since 2004 90 clinics with 11 clinics in FL 80,000 + members
Would involve the development of an on sight clinic owned by the county and
managed by Care Here
Reciprocal arrangement with PSL could be made (PSL claims trend went to less
than 1%)
$20,000 supply and equipment
$23.00 per /employee per/mo ($378,386 based on employee level)
$692,705 annual cost
No per visit fee
$952,203 cost avoidance (net after build-out and start up costs)
Employs a full time Pharmacy Director
Physicians Immediate Care
Started in 2003 currently have three local urgent care facilities in the area soon
to be opening a fourth in SLW in April.
No upfront costs
Would utilize three urgent care centers in the county
$160 per/visit flat fee package of services
$681,526 annual cost avoidance to the county
$23.00 per employee/per mo
$378,000 estimated annual cost
$692,706 staff supplies, Rx
Staff not dedicated to County employees
Cost/Savings Summary
• 1St Year projected net savings after operating cost $1,102,203
• Estimated facility build out of $150,000
• New savings estimate after build out costs $952,203
• 3 year projected net savings after clinic costs $7,197,913
Additional clinic saving
• 1. Annual projected employee co-pay savings $268,000
• 2. BC/BS administrations savings (3yrs) $75,000
The Human Resource Director was in favor of the employee clinic concept after reviewing the
information presented.
Mr. Davis concurred with the Human Resource Director and based his preference on the facts
regarding the Port St. Lucie clinic that had illustrated the less than 1% rise in claims.
Commissioner Dzadovsky recommends having a reciprocal agreement with Port St. Lucie. He
stated he would like to be analyzing real numbers in a year and then comparing them.
There was consensus of the entire Board in favor of the employee clinic concept.
Senate Bill 1216
There was a unanimous consensus to support Commissioner Lewis. A resolution will be
discussed at the next meeting of the Board.
Harbor Advisory
Gerald Koklinski, Chairman of the Harbor Advisory committee made an engineering
presentation to the Board.
He stated that the entrance to Harbor Pointe (previously called the Cotton Property) is
currently not user friendly. Recreational and tourist usage was its purpose.
He proposed that the County purchase the Right of Way from the edge end of the Bell property
to Harbor Pointe. Next, he would recommend we purchase 5 acres up to the east of Cracker
Boy from Mr. Bell so, boat ramps and docks can be added.
Commissioner Coward stated that it was difficult, if not near impossible to get Mr. Bell to sell
anything to the County. He stated basing a proposal on Mr. Bell's cooperation would be moot.
Commissioner Dzadovsky wanted to apprise the Board that a pitch was made to Mr. Bell by
longshoreman and port interests to expand the port, go against the referendum, and deepen
the inlet for cargo operations.
Mr. Kolklinski responded he was unaware of this and then inquired if anyone knew if Mr. Bell
signed a letter of intent. If he hasn't signed a letter of intent then that would be a moot point.

The County Administrator stated since her appointment last year, she has had three separate
meetings with Mr. Bell where he presented three different contracts he was considering and to
date, none has materialized. She felt certain he would not be willing to sell.
The Chairman stated that if we can get an offer down for just the parking area and adjacent
boat ramp from Mr. Bell, we may have an answer.
Chairman Kolkinski urged the Board to consider utilizing the existing Harbor Point purchase for
a cultural arts center in the future when funding could be made available.
The County Admin stated that some ideas like a boardwalk and picnic facilities were being
looked into with possible funding coming from grants.
Commissioner Coward agreed that everyone wants to see something done with this property
and feels it's unfortunate that the seller is not cooperating. However, there is no money or
staff dedicated for new projects at this time, therefore he recommends we forward to our grant
writers to see what they can come up with for the long term
