DISCIPLINARY CASE: 1) AGAINST: Chase Home Repairs & Improvement LLC Frank Chase County Certificate # 28820 Complaint filed by: Daniel Hokeness, property owner of 10600 S Ocean Dr., Unit 607 Investigator: Monica Vargas Barrios First Notification to the Company: December 23, 2015 Follow-ups: Letters and phone calls. Violation: SLC CCL 10-23 Display of State Registration and County Certificate numbers. SLC CCL 10-26 Prohibited Activities. Sub section: (1) – Falsely hold himself out as a licensed contractor. Sub section: (9) – Commit any willful, fraudulent act as a contractor as a result of which another is financially injured. Sub section: (12) – Commence or perform work for which a county building permit is required without obtaining the appropriate permit. SLC CCL 10-67: Revocation or suspension of certificates of competency. Sub section: (3) Willfully disregards and violates an applicable state or local building code, regulation or law. Sub section: (7) Fails in any material respect to comply with the provisions of this division. Sub section: (10) Commits fraud or deceit or other misconduct in the practice of contracting. Sub Section: (11) Proceeds on any construction job without first obtaining and posting the required county building permits and scheduling required inspections. Sub section: (12) Exceeds the scope of work which the contractor has been licensed to perform. Staff Recommendation: To revoke the Certificate of Competency not to exceed one (1) year. It is further recommended that this case along with the copy of this Board’s Order be prepared and sent to the surrounding municipalities. Page 4 of 4 K. OLD BUSINESS:  Discussion on comparison of the pass/fail rate of all three (3) testing agencies GITS, Prometric, and ProV. M. NEW BUSINESS:  Discussion on drafting a new policy
